摘  要:目的探讨改良Galveston技术结合3D打印治疗垂直不稳定骨盆骨折(Tile C3)的效果及临床意义。方法2017年3月至2021年6月收治Tile C3型骨盆骨折患者82例,其中42例采用改良Galveston手术技术结合3D打印技术进行固定(观察组),40例采用传统手术方法固定(对照组)。比较两组患者的手术时间、麻醉时间、术中出血量、术中透视次数、术后骨折复位质量、术后下地及负重时间、术后功能评价。TornettaMatta标准评价骨折复位质量,根据Majeed评分记录功能优良率。结果所有患者均获随访,均无内固定松动及神经、血管损伤等并发症发生,随访时间为1、2、3、6、12个月。观察组患者术中平均手术时间短、术中透视次数少,两组间比较差异均有统计学意义(P均<0.05)。两组患者术中麻醉时间、术中出血量比较差异均无统计学意义(P均>0.05)。根据Tornetta-Matta标准及Majeed评分(末次随访)评价,两组骨折复位质量及骨盆功能恢复情况差异均无统计学意义(P均>0.05)。结论改良Galveston技术结合3D打印治疗Tile C3型骨盆骨折手术操作时间短,术中透视次数少,固定强度高,可早期下地及负重,较早恢复肢体功能。Objective To explore the effect and clinical significance of modified Galveston technology combined with 3D printing in the treatment of vertically unstable pelvic fractures(Tile C3).Methods From March 2017 to June 2021,82 cases of Tile C3 pelvic fracture were treated.Among them,42 cases were fixed by modified Galveston surgical technique combined with 3D printing technique(observation group),and 40 cases were fixed by traditional surgical method(control group).The operation time,anesthesia time,intraoperative blood loss,times of X-ray fluoroscopy,postoperative reductive quality of fracture,postoperative landing time and weight-bearing time,postoperative functional evaluation were compared between the two groups of patients.The Tornetta-Matta standard was used to evaluate the quality of fracture reduction,and the excellent and good rate of function was recorded according to the Majeed score.Results All patients were followed up.There were no complications such as internal fixation loosening and nerve and blood vessel damage.The follow-up time was 1,2,3,6 and 12 months.In observation group,the average operation time was short and the number of intraoperative fluoroscopy was less.The difference between the two groups was statistically significant(P all<0.05).There was no significant difference in the statistical data of intraoperative anesthesia time and intraoperative blood loss between the two groups of patients(P all>0.05).No statistical differences was found in anesthesia time,blood loss between two groups(P all>0.05).According to the evaluation of Tornetta-Matta standard and Majeed score(the final follow-up),the two groups showed no significant difference whether in the clinical results of fracture reduction or the recover of pelvic function(P all>0.05).Conclusion The modified Galveston technology combined with 3D printing for the treatment of vertically unstable pelvic fractures(Tile C3)has the advantages of reducing the operative time and the frequency of X-ray,strengthening fixation,making an early ambulati
