培哚普利联合左旋氨氯地平对高血压合并左心室肥厚患者的临床价值分析

摘　　要：目的探究培哚普利联合左旋氨氯地平对高血压合并左心室肥厚(left ventricular hypertrophy,LVH)患者血浆脑钠肽(BNP B-type natriuretic peptide)等生化指标、临床疗效的影响。方法选取于2018年1~12月来笔者医院接受治疗的高血压合并左心室肥厚患者91例作为研究对象。通过倾向评分匹配后,随机数字法将其分为两组,对照组32例和实验组32例。两组患者均进行常规治疗,包括合理膳食、适当运动、戒烟、戒酒等。对照组在常规治疗基础上口服左旋氨氯地平片治疗,实验组在规治疗基础上上口服培哚普利、左旋氨氯地平片治疗。通过血浆同型半胱氨酸(Hcy homocysteine)、心钠肽(ANP atrial natriuretic peptide)、脑钠肽(BNP B-type natriuretic peptide)水平;临床疗效两方面进行对比。结果治疗后,实验组和对照组血浆Hcy、ANP、BNP水平均低于治疗前,实验组低于对照组,差异有统计学意义(P<0.05)。治疗后,对照组高血压合并LVH患者临床总有效率分别为78.13%,实验组为93.75%,实验组高于对照组,差异有统计学意义(P<0.05)。结论培哚普利联合左旋氨氯地平治疗高血压合并左心室肥厚疗效显著,可有效降低血浆BNP等生化指标水平、改善左室收缩和舒张功能,提高临床疗效,具有一定临床推广价值。Objective To explore the effect of perindopril combined with levamlodipine on plasma BNP and other biochemical indicators and clinical efficacy in patients with hypertension complicated with left ventricular hypertrophy(LVH).Methods Ninety-one patients with hypertension and LVH who came to our hospital for treatment from January 2018 to December 2018 were selected as the research objects.After matching by propensity score,they were divided into 2 groups by random number method,32 cases in the control group and 32 cases in the experimental group.Patients in both groups received conventional treatment,including reasonable diet,appropriate exercise,quitting smoking and alcohol,etc.The control group was treated with oral L-Amlodipine tablets on the basis of conventional treatment,and the experimental group was treated with Perindopril and L-Amlodipine tablets on the basis of regular treatment.The clinical efficacy of two aspects were compared through plasma Hcy,ANP,BNP levels.Results After treatment,the plasma Hcy,ANP,and BNP levels of the experimental group and the control group were lower than those before the treatment,and the experimental group was lower than the control group(P<0.05),and the difference was statistically significant.After treatment,the total clinical effective rate of patients with hypertension and LVH in the control group was 78.13%,and the experimental group was 93.75%.The experimental group was higher than the control group(P<0.05),and the difference was statistically significant.Conclusion Perindopril combined with L-Amlodipine has a significant effect on the treatment of hypertension with left ventricular hypertrophy.It can effectively reduce the level of plasma BNP and other biochemical indicators,improve left ventricular systolic and diastolic function,and improve clinical efficacy.It has certain clinical promotion value.
