基于数据挖掘分析“参元益气活血胶囊”在刘红旭教授治疗稳定性心绞痛方药体系中的运用情况

摘　　要：目的 分析“参元益气活血胶囊(SYYQ)”在治疗稳定性心绞痛(Stable angina pectoris, SAP)中的运用规律。方法 通过数据统计,关联规则及系统聚类对首都医科大学附属北京中医医院刘红旭教授门诊近10年的SAP处方数据进行分析,并与既往研究中其治疗经皮冠状动脉治疗术(PCI)术后1个月内的用药规律进行对比。结果 本研究共纳入方剂3214首,涉及药物277味,药物使用频次54 443次;SYYQ方中药物使用频率:生黄芪(76.1%)、延胡索(61.8%)、丹参(34.7%)、水蛭(33.5%)、玄参(21.3%)、党参(20.9%)、土鳖虫(20.1%)、地龙(16.2%);关联规则中“提升度”前10的药对中,由SYYQ方中药物组成药对占5对;系统聚类分析发现SYYQ方中药物主要分布在C1类;与SYYQ治疗PCI术后1个月内患者的用药特征相比,SYYQ治疗SAP在药物使用频率上具有其自身的潜在特点。结论 SYYQ在整体治疗SAP的方药体系中具有使用频次高,药物间关联性强,同步性好以及随冠心病病证不同阶段,不同严重程度加减变化的特点,是刘红旭教授治疗稳定性心绞痛的核心方之一。Objective To analyze the application regularity of ShenyuanYiqiHuoxue Capsules(SYYQ)in the treatment of stable angina pectoris(SAP).Methods The data of prescriptions against SAP by Professor Liu Hongxu from Beijing Hospital of Traditional Chinese Medicine Affiliated to Capital Medical University in the past 10 years were analyzed by data statistics, association rules, and hierarchical clustering, and compared with medication regularity one month after PCI in previous studies.Results A total of 3214 prescriptions were included, involving 277 drugs, and the frequency of drug use was 54 443.In terms of frequency of drug use in SYYQ,the drugs were ranked as Astragali Radix(76.1%),Corydalis Rhizoma(61.8%),SalviaeMiltiorrhizae Radix et Rhizoma(34.7%),Hirudo(33.5%)Scrophulariae Radix(21.3%),Codonopsis Radix(20.9%),EupolyphagaSteleophaga(20.1%),and Pheretima(16.2%).Among the top 10 drug pairs in the "lift" in the association rules, the drugs in SYYQ composed five pairs.Hierarchical systematic clustering analysis found showed that the drugs in SYYQ were mainly distributed in the C1 category.Compared with the medication characteristics of SYYQ for patients within one month after PCI,SYYQ against SAP displayed potential characteristics in the frequency of drug use.Conclusion SYYQ has the characteristics of a high frequency of use, strong correlation between drugs, good synchronization, and appropriate modificationalong with different stages and different conditions of coronary heart disease syndromes in the overall treatment of SAP.SYYQ is one of the core prescriptions by Professor Liu Hongxu in the treatment of SAP.
