清热化瘀法对郁结内热型不明原因复发性流产血栓前状态、外周血炎性因子及T淋巴细胞亚群水平的影响

摘　　要：目的:探讨清热化瘀法对郁结内热型不明原因复发性流产(URM)血栓前状态和外周血炎性因子以及T淋巴细胞亚群水平的影响。方法:收集郁结内热型URM患者30例和正常女性30例,检测外周血清D二聚体(D-D)、纤维蛋白降解产物(FDP)、纤维蛋白原(FIB)、IL-6、IL-10及TNF-α、CD_(3)^(+)、CD_(4)^(+)、CD_(8)^(+)、CD_(19)^(+)和CD_(56)^(+)水平变化特点,对URM患者用清热化瘀中药干预3个月经周期,观察治疗前后指标变化情况。结果:与正常女性比较,郁结内热型URM患者外周血清D-D、IL-6、TNF-α和CD5+6含量增高(P<0.05),而IL-10淋巴细胞含量显著降低(P<0.05);与治疗前比较,治疗3个月经周期后D-D、IL-6、TNF-α和CD_(56)^(+)含量下降(P<0.05),IL-10和CD_(4)^(+)T淋巴细胞含量显著提高(P<0.05)。结论:清热化瘀法可有效改善URM血栓前状态,作用机制可能与调节免疫和外周血炎性因子变化有关。Objective:To investigate the effects of clearing heat and removing blood stasis on the pre-thrombotic state,peripheral blood inflammatory factors and T lymphocyte subsets in unexplained recurrent abortion(URM)with suppressed internal heat type.Methods:A total of 30 URM patients and 30 normal women were collected to detect the changes of D dimer(D-D),fibrin degradation products(FDP),fibrinogen(FIB),IL-6,IL-10,TNF-α,CD_(3)^(+),CD_(4)^(+),CD_(8)^(+),CD_(19)^(+) and CD_(56)^(+) in serum.URM patients were treated with Chinese medicine to clear heat and remove stasis for 3 menstrual cycles,and the changes of indexes before and after treatment were observed.Results:The contents of D-D,IL-6,TNF-αand CD5+6 in peripheral blood of URSA with suppressed internal heat were increased,while the content of IL-10 was decreased,and the differences were statistically significant compared with those of normal women(P<0.05).Compared with before treatment,after 3 menstrual cycles,the levels of D-D,IL-6,TNF-α and CD_(56)^(+) were decreased(P<0.05),while the levels of IL-10 and CD_(4)^(+)T were significantly increased(P<0.05).Conclusion:Clearing heat and removing blood stasis can effectively improve the pre-thrombotic state of URM,and the mechanism may be related to regulating the changes of immune and peripheral blood inflammatory factors.
