FTS理念围术期护理干预在经尿道输尿管镜碎石取石术患者中的应用

摘　　要：目的:探讨快速康复外科(FTS)理念围术期护理干预在经尿道输尿管镜碎石取石术患者中的应用效果。方法:将2019年1月1日~12月31日收治的126例经尿道输尿管镜碎石取石术患者随机分为对照组和研究组各63例,对照组实施常规护理干预,研究组实施FTS理念围术期护理干预;比较两组负性心理[采用焦虑自评量表(SAS)和抑郁自评量表(SDS)]、饥饿及口渴情况[采用数字评分法(NRS)]、术后恢复时间、疼痛程度[采用视觉模拟评分法(VAS)]、并发症发生情况、早期恢复质量[采用15项恢复质量评分量表(QoR-15)]。结果:研究组SAS、SDS、饥饿、口渴评分均低于对照组(P<0.01);研究组术后首次排气时间、下床活动时间及住院时间均短于对照组(P<0.01);研究组VAS评分低于对照组(P<0.01);研究组并发症总发生率低于对照组(P<0.05);研究组生理舒适度、生理独立性、心理支持、情感评分均高于对照组(P<0.01)。结论:在经尿道输尿管镜碎石取石术患者围术期护理中应用FTS理念,可改善患者负性心理,优化患者术后恢复指标,减轻疼痛,降低并发症发生率,提高患者早期恢复质量。Objective:To explore the application effect of the rapid rehabilitation surgery(FTS)concept perioperative nursing in patients with transurethral ureteroscopy lithotripsy removal.Methods:A total of 126 cases of patients with transurethral ureteroscopy lithotripsy removal admitted during January 1 to December 31,2019 were randomly divided into the control group and the study group withe 63 cases each,the control group implemented routine nursing intervention,the study group implemented FTS concept perioperative nursing intervention.Comparison was made between the two groups on negative psychology by the self-assessment scale on anxiety(SAS)and the self-rating scale on depression(SDS),hunger and thirst by the numerical ratng scale(NRS),postoperative recovery time,pain degree by visual analogue scale(VAS),complications,early recovery quality by the 15 restoring quality rating scale(QoR-15).Results:The scores of SAS,SDS,hunger,thirst were lower in the study group than those in the control group(P<0.01).The initial postoperative exhaust time,ambulation time and hospital stay were shorter in the study group than those in the control group(P<0.01).The VAS scores were lower in the study group than those in the control group(P<0.01).The total incidence of complication was lower in the study group than that in the control group(P<0.05).The physical comfort,physiological independence,psychological support,emotional scores were higher in the study group than those in the control group(P<0.01).Conclusion:Patients with transurethral ureteroscopy rubble stone extraction for Application of the rapid rehabilitation surgery(FTS)concept perioperative nursing in patients with transurethral ureteroscopy lithotripsy removal,can improve their negative psychology,optimize their postoperative recovery indicators,relieve pain,reduce the incidence of complications,and improve their quality of early recovery.
