心率变异阈值评价肥胖儿童通气阈的探索研究

摘　　要：为探索和验证肥胖儿童在运动干预前后心率变异阈值(HRVT)评价通气阈(VT)的有效性,23名肥胖儿童在6周运动干预前后分别完成递增负荷运动心肺功能测试,采用心率表和气体代谢分析仪测定HRVT与VT,检验HRVT与VT对应心率(HR)和功率(WP)的相关性与一致性.结果发现:HRVT和VT对应的强度判断指标HR与WP具有高重测信度;HRVT与VT对应的HR与WP呈显著正相关,其中频域分析方法获得的HRVT与VT呈现高相关;HRVT与VT对应强度判断指标的一致性高,均值偏差趋于0,95%一致性界限的置信区间较窄.可见,肥胖儿童在6周运动干预前后的HRVT与VT具有较好的一致性和稳定性,HRVT是有效评价肥胖儿童VT的新方法.In order to explore and verify the effectiveness in assessment of ventilatory threshold(VT)using heart rate variability threshold(HRVT)in obese children before and after exercise intervention,twenty-three obese children were enrolled in the research.Before and after 6-week exercise intervention,the participants performed incremental exercise cardiopulmonary test.HRVT and VT were measured by heart rate monitor and gas metabolism analyzer to examine the correlation and consistency with heart rate(HR)and work power(WP).The results showed that HR and WP,the strength judgment indexes corresponding to HRVT and VT,had high test-retest reliability.HRVT and VT were significantly positively correlated with the corresponding HR and WP,in which the HRVT obtained by frequency domain analysis method showed a high correlation with VT.The consistency between HRVT and VT was high.The mean bias tended to zero,and the confidence interval of 95%limited of agreement was narrow.It was concluded that HRVT and VT had an acceptable consistency and stability in obese children before and after 6-week of exercise intervention.HRVT could be a new effective method to evaluate VT in obese children.
