摘  要:目的应用放射组学分析腹主动脉瘤腔内修复术(EVAR)后Ⅱ型内漏的发生与腰动脉的关系。方法收集2015年1月至2021年1月大庆油田总医院收治的147例腹主动脉瘤患者的临床资料。应用放射组学对患者术前计算机断层扫描血管造影(CTA)影像数据进行提取,同时结合临床数据探讨腹主动脉瘤EVAR术后Ⅱ型内漏的发生与腰动脉情况的关系。结果通过对放射组学特征进行分析发现,11例Ⅱ型内漏患者的平均腰动脉数目为5条,最大腰动脉直径为2.1 mm。136例未发生内漏患者的平均腰动脉数目为2条,最大腰动脉直径为1.4 mm。发生Ⅱ型内漏与未发生内漏腹主动脉瘤患者的平均腰动脉数目、最大腰动脉直径比较,差异均有统计学意义(P<0.05)。不同年龄、性别腹主动脉瘤患者的术后Ⅱ型内漏发生情况比较,差异均无统计学意义(P>0.05)。结论应用放射组学预测腹主动脉瘤EVAR术后Ⅱ型内漏发生与腰动脉的关系是可行的。Objective To explore the relationship of type Ⅱ endoleak with lumbar artery after endovascular aneurysm repair(EVAR) of abdominal aortic aneurysm by using radiomics. Method The clinical data of 147abdominal aortic aneurysm patients in Daqing Oilfield General Hospital from January 2015 to January 2021 were collected, and preoperative computed tomography angiography(CTA) data were extracted by radiomics. At the same time, combined with clinical data, the the relationship of type Ⅱ endoleak with lumbar artery after EVAR of abdominal aortic aneurysm was discussed. Result The results of analyzed features of radiomics showed that the average number of lumbar arteries was 5, with a maximum lumbar artery diameter of 2.1 mm in 11 patients with type Ⅱ endoleak. The average number of lumbar arteries in 136 patients without endoleak was 2, with a maximum lumbar artery diameter of 1.4 mm. In patients with abdominal aortic aneurysm, there were significant differences in the average number and maximum diameter of lumbar artery between patients with type Ⅱ endoleak and those without endoleak(P<0.05);there was no significant difference for occurrence of type Ⅱ endoleak in patients with different age and sex(P>0.05).Conclusion It is feasible to apply radiomics to predict the relationship of type Ⅱ endoleak with lumbar artery after EVAR of abdominal aortic aneurysm.
