摘　　要：目的探讨下肢穿支静脉功能不全(IPV)患者彩色多普勒超声特征与其临床分级的相关性。方法收集2018年7月至2020年6月威海市立医院收治的60例IPV患者的临床资料,使用彩色多普勒超声检测患者的穿支静脉功能状态,分析不同临床表现-病因学-解剖学-病理生理学(CEAP)分级与IPV的相关性。结果60例患者72条患肢中,共检出IPV的静脉187条,其中24条患肢伴有皮肤营养改变或溃疡(C5/6级),占比43.06%。24条患肢中共检出IPV的静脉99条,占比52.94%,其中19.19%存在穿支静脉持续性反流。不同CEAP分级与IPV数目(ρ=0.818,P<0.05)、平均内径(ρ=0.950,P<0.05)、反流时间(ρ=0.911,P<0.05)均呈正相关。结论彩色多普勒超声检查明确了IPV的解剖情况和血流动力学状态,且其超声特征还能与患者CEAP分级相互印证,给手术治疗提供了导向,为临床治疗方案的制定和疾病的预后提供参考,值得推广应用。Objective To investigate the correlation between color Doppler ultrasound features and clinical grades in patients with lower extremity incompetent perforator vein(IPV).Method The clinical data of 60 patients with IPV admitted to Weihai Municipal Hospital from July 2018 to June 2020 were collected.Color Doppler ultrasonography was used to detect the functional status of the perforating veins of the patients.The correlation between different clinical etiology anatomy pathophysiology(CEAP)grades and IPV was also analyzed.Result Among 72 extremities of 60 patients,187 IPV were detected,of which 24 extremities were accompanied by skin nutritional changes or ulcers(C5/6 grade),accounting for 43.06%.A total of 99 IPV were detected in 24 affected extremities,accounting for 52.94%,of which 19.19% had persistent perforator vein reflux.Different CEAP grades were positively correlated with IPV number(ρ=0.818,P<0.05),average inner diameter(ρ=0.950,P<0.05),and reflux time(ρ=0.911,P<0.05).Conclusion Color Doppler ultrasonography clarifies the anatomy and hemodynamic status of IPV,and its ultrasonographic features can also corroborate with the patient's CEAP classification,which provides guidance for surgical treatment,as well as the formulation of clinical treatment plans and diseases.Therefore,it is worthy of popularization and widespread application.
