摘　　要：目的应用关联规则数据挖掘技术,研究清溃方治疗轻中度活动期溃疡性结肠炎配伍规律,并评价其临床疗效。方法收集127例在上海中医药大学附属龙华医院单纯使用中药健脾清肠法治疗轻中度活动期溃疡性结肠炎(UC)患者治疗8周的组方共508条,建立方剂和药物数据库,分析健脾清肠法的组方规律,运用现代数据挖掘技术提取清溃方,并将提取的组方加减分别于上海中医药大学附属上海市中西医结合医院、龙华医院、光华中西医结合医院进行多中心回顾性研究,收集93例单纯使用组方加减治疗8周的轻中度活动期UC患者,分析临床疗效。结果通过关联分析,筛选出健脾清肠法治疗轻中度UC中出现频率及支持率占高位的药物,组成清溃方。该组方对改善UC患者临床证候总有效率为91.40%;其中45例近期复查肠镜的UC患者,结肠黏膜改善降1级占57.78%,降2级占8.89%。结论清溃方是治疗轻中度活动期UC(脾虚湿热证)的有效方,值得临床推广应用。Objective:To study the compatibility law of Qingkui Prescription in the treatment of mild-to-moderate active ulcerative colitis(UC)patients by using association rule data mining technology,and to evaluate its clinical effect.Methods:508 prescriptions were collected from 127 patients with mild-to-moderate active UC,who were treated with Qingkui Prescription in Longhua Hospital affiliated to Shanghai University of Traditional Chinese Medicine for 8 weeks,and a database of prescriptions and drugs was established.The prescription rules of Qingkui Prescription were analyzed by using modern data mining technology to extract Qingkui prescription.A multi-center retrospective study of the modified extracted prescriptions was respectively conducted in Shanghai Hospital of Integrated Traditional Chinese and Western Medicine,Longhua Hospital,and Guanghua Traditional Chinese and Western Medicine was conducted.93 patients who were treated with the modified prescriptions for 8 weeks were collected,and their clinical effect was analyzed.Results:Through the correlation analysis,the drugs with high frequency and support rate in the treatment of mild-to-moderate UC with Jianpi Qingchang method were screened out to form Qingkui Prescription.The total effective rate of this prescription in improving the clinical syndromes of UC patients was 91.40%;among 45 UC patients who had recently undergone colonoscopy,the colonic mucosa improved by one grade accounted for 57.78%,and the colon mucosa was reduced by two grades accounted for 8.89%.Conclusion:Qingkui Prescription is an effective prescription for the treatment of mild-to-moderate active UC patients(spleen deficiency and damp-heat syndrome),worthy of clinical application.
