常规磁共振成像联合动脉自旋标记在儿童胚胎发育不良性神经上皮瘤诊断中的价值

摘　　要：目的探讨常规磁共振成像(MRI)联合动脉自旋标记(ASL)灌注成像在儿童胚胎发育不良性神经上皮瘤(DNET)中的诊断价值。方法回顾性分析2015年1月至2020年1月经病理证实的7例DNET患者,观察肿瘤常规MRI表现及ASL灌注特征。结果DNET以颞叶多见,所有病灶均呈T1WI低信号、T2WI高信号,楔形或扇形形态、囊内分割线、T2液体衰减反转恢复序列(T2FLAIR)序列上环状高信号、颅骨受压变薄均有出现。瘤体无强化或有实性结节轻度强化,瘤周无水肿,可使中线向对侧推移。肿瘤表观扩散系数(ADC)的值明显低于对侧正常白质(P<0.001),ASL呈低灌注或出现稍高灌注结节。结论DNET具有特征性MRI表现,结合ASL技术可进一步提高诊断的准确率。Objective To investigate the value of conventional magnetic resonance imaging(MRI)combined with arterial spin labeling(ASL)perfusion imaging in children with dysembryoplastic neuroepithelial tumor(DNET).Methods A retrospective analysis was made of 7 DNETs confirmed by pathology,and their manifestations of conventional MRI and perfusion characteristics of ASL were observed.Results DNET was commonly seen in the temporal lobe,all lesions presented low T1WI signal and high T2WI signal,and wedge-shaped or fan-shaped shape,intrcapsular division line,high annular signal on T2FLAIR sequence,and thinly compressed skull could been seen.There was no enhancement of the tumor or mild enhancement of solid nodules in the tumor,no edema around the tumor,and the midline could be pushed to the opposite side by the tumor.The apparent diffusion coefficient value of the tumor was significantly lower than that of the contralateral normal white matter(P<0.001).ASL presented low perfusion or showed nodules with slightly high perfusion.Conclusion DNET has characteristic MRI performance,and the combination of ASL technology can further improve the diagnostic accuracy.
