摘　　要：目的探究急性心肌梗死患者血清脂蛋白a[Lp(a)]、血清钙卫蛋白A4(S100A4)表达水平与血小板活化的关系及对心脏不良事件的影响。方法选取我院100例急性心肌梗死(acute myocardial infarction,AMI)患者为研究组,116例健康体检者为对照组,检测血清Lp(a)、S100A4和血小板活化物指标。对患者随访1年,分析患者发生心脏不良事件的危险因素。结果研究组血清中Lp(a)、S100A4和血小板活化物水平均高于对照组(P<0.05)。AMI患者在1年内出现心脏不良事件42例。不良事件组血清Lp(a)和S100A4水平均偏高。对患者行非条件logistic回归分析显示,Lp(a)、S100A4均是影响AMI患者发生心脏不良事件的危险因素。结论血清Lp(a)、S100A4表达水平越高,患者发生心脏不良事件的风险越高,对其有一定预测价值。Objective To investigate the relationship of serum lipoprotein a(Lp(a))and S100 calcium binding protein A4(S100A4)expression level with platelet activation in patients with acute myocardial infarction(AMI)and its influence on major adverse cardiac events.Methods One hundred AMI patients and another 116 healthy individuals in our hospital were enrolled as study group and control group.LP(a),S100A4 and platelet activation related parameters were assessed,and all patients completed one-year follow-up.Risk factors affecting the occurrence of major adverse cardiac events in AMI patients were screened.Results Serum level of Lp(a),S100A4 and platelet activation related parameters were significantly higher in study group than those in control group,showing statistical difference(all P<0.05).Major adverse cardiac events occurred in 42 AMI patients within 1 year.Serum Lp(A)and S100A4 level was abnormally elevated in patients with major adverse cardiac events.Unconditional logistic regression analysis showed that Lp(A)and S100A4 were risk factors affecting the occurrence of major adverse cardiac events in patients with AMI.Conclusion Serum Lp(a)and S100A4 expression level is positively correlated with platelet activation related parameters,indicating AMI patients with higher serum Lp(A)and S100A4 level suffer a greater risk of major adverse cardiac events.
