摘　　要：目的探讨腹腔镜下腹股沟疝全腹膜外网片修补术对腹股沟疝患者炎性反应、慢性疼痛发生情况的影响。方法选取2018年7月至2020年6月我院接收的86例腹股沟疝患者进行研究,以抽签法将其分为对照组与观察组,每组43例。对照组采取传统无张力疝修补术,观察组实施腹腔镜下腹股沟疝全腹膜外网片修补术。比较两组的手术相关指标、炎性反应指标、疼痛程度及慢性疼痛发生情况。结果观察组的术中出血量少于对照组,手术时间、术后下床活动时间、住院时间均短于对照组(P<0.05)。术后,观察组的中性粒细胞百分比、白细胞计数高于对照组,CRP水平低于对照组(P<0.05)。观察组术后4、7、14 d的VAS评分均低于术后1 d及对照组(P<0.05)。术后6个月经随访,观察组的慢性疼痛发生率低于对照组(P<0.05)。结论腹腔镜下腹股沟疝全腹膜外网片修补术用于腹股沟疝患者治疗中,不仅可缓解机体的炎性反应,还能改善患者的疼痛程度,降低慢性疼痛发生率,值得推广。Objective To investigate the effects of laparoscopic total extraperitoneal herniorrhaphy on inflammatory reaction and the occurrence of chronic pain in patients with inguinal hernia.Methods A total of 86 patients with inguinal hernia received in our hospital from July 2018 to June 2020 were selected for study.The patients were divided into control group and observation group by lottery,with 43 cases in each group.The control group was treated with traditional tension-free hernia repair,and the observation group was treated with laparoscopic total extraperitoneal herniorrhaphy.The operation related indexes,inflammatory reaction indexes,degree of pain and occurrence of chronic pain were compared between the two groups.Results The amount of intraoperative bleeding in the observation group was less than that in the control group,and the operation time,postoperative out of bed activity time and hospital stay were shorter than those in the control group(P<0.05).After operation,the percentage of neutrophils and the counts of leukocytes in the observation group were higher than those in the control group,and the level of CRP was lower than that in the control group(P<0.05).The VAS score of the observation group at 4,7 and 14 d after operation was lower than that at 1 d after operation and the control group(P<0.05).Follow-up of 6 months after operation,the incidence of chronic pain in the observation group was lower than that in the control group(P<0.05).Conclusion Laparoscopic total extraperitoneal herniorrhaphy applied in the treatment of patients with inguinal hernial can not only alleviate the inflammatory reaction of the body,but also improve the degree of pain and reduce the incidence of chronic pain,which is worth popularizing.
