睾丸扭转患儿及时就诊的影响因素分析

摘　　要：目的探讨影响睾丸扭转(TT)患儿及时就诊的因素。方法选取2015年1月-2021年7月于阜阳市人民医院就诊的120例TT患儿及其家长作为研究对象,根据患者发病至来我院就诊的时间分为≤6 h组、6~24 h组、>24 h组。通过查阅临床病历及电话回访的方法搜集患儿及家长的相关资料,应用多因素logistic回归分析探索影响睾丸扭转患儿及时就诊的相关因素。结果120例患儿年龄5~17岁,平均(10.26±2.29)岁;≤6 h组67例,6~24 h组40例,>24 h组13例。3组间的睾丸保留率比较差异具有统计学意义(P<0.05)。单因素分析发现父母职业、医疗保险、TT发生的时间、阴囊急症相关教育、离医院的距离、阴囊疼痛的程度、意识到发生TT均是影响及时就诊的因素,差异具有统计学意义(P<0.05);父母的年龄、经济收入及阴囊不适为首发症状不是影响及时就诊的因素(P>0.05)。多因素logistic回归分析显示父母职业为医学相关、有医疗保险、睾丸扭转发生的时间在日间且≤6 h、父母受过阴囊急症相关教育、离医院的距离<10 km、阴囊疼痛剧烈、意识到发生TT均是影响患儿及时就诊的独立保护因素(P<0.05)。结论TT及时寻求医疗干预受多重因素的影响,我们应该加大TT相关知识方面的宣传,强调及时就诊的必要性,以提高公众对TT的认识。Objective To explore factors influencing the timely attendance of children with testicular torsion(TT).Methods A total of 120 children with TT treated during Jan.2015 and Jul.2021 were divided into three groups according to the time between onset and arrival at hospital:≤6 h group,6~24 h group and>24 h group.The factors affecting the timely consultation were analyzed with multivariate logistic regression.Results All 120 children aged 5 to 17 years,mean(10.26±2.29)years,including 67 cases in the≤6 h group,40 in the 6~24 h group and 13 in the>24 h group.There was significant difference in testicular preservation rate among the groups(P<0.05).Univariate analysis revealed that parents occupation,health insurance,time of onset,education related to scrotal emergencies,distance to hospital,degree of scrotal pain,and awareness of TT were factors influencing timely consultation(P<0.05);parental age,financial income,and scrotal discomfort as the first symptom were not factors influencing timely consultation(P>0.05).Multivariate logistic regression showed that parents occupation,health insurance,time of onset,education related to scrotal emergencies,distance to hospital,degree of scrotal pain,and awareness of TT were independent factors influencing timely attendance(P<0.05).Conclusion Seeking timely medical intervention for testicular torsion is influenced by multiple factors,and public awareness should be increased for prompt medical attention.
