巢国俊从“气血”论治非动脉炎性前部缺血性视神经病变经验

摘　　要：非动脉炎性前部缺血性视神经病变(NA-AION)是指营养视神经的血液循环障碍导致的视神经急性缺血性病变,多发于中年以上患者。其主要由于供应视盘筛板区的睫状后短动脉缺血所致,表现为突然视力障碍和眼底视盘水肿。本文介绍了巢国俊教授从“气血”角度辨析NA-AION的病因及发病机制,认为本病的发生主要由于气血瘀滞所致,治疗时宜以养血活血为主,同时兼顾补益肾精、益气利水和清热散结等治法。临床采用桃红四物汤加减方灵活加减运用治疗本病取得较好疗效。Non-arteritic anterior ischemic optic neuropathy(NA-AION) refers to the acute ischemic optic neuropathy caused by the disturbance of blood circulation that nourishes the optic nerve, which mostly occurs in the patients above middle age. It is mainly caused by ischemia of the posterior ciliary short artery supplying the ethmoid plate area of the optic disc, characterized by sudden visual impairment and fundus optic disc edema. This paper introduces Professor CHAO Guojun’s analysis of the etiology and pathogenesis of NA-AION with Qi and Blood Theory. It is considered that the disease is mainly caused by Qi and blood stasis. The doctor should focus on nourishing blood and promoting blood circulation, while taking into account the treatment methods of tonifying kidney essence, supplementing Qi, dredging water, clearing away heat and dispersing knot. The clinical application of Taohong Siwu Decoction in the treatment of NA-AION has achieved good curative effect.
