脾主升清理论在干眼论治中的运用探讨

摘　　要：结合干眼的临床特征,佐以现代临床运用与研究,提出基于脾主升清理论治疗干眼的理法方药。干眼的发生多由目窍不得精微物质濡养所致,而五脏六腑之精气上注于目需脾主升清以实现。脾为后天之本,气血化生之源,五脏气血精微通过脾气枢转,输注于目,从而发挥滋养目窍功能,以阻断干眼发生。在脾主升清防治干眼的过程中,若目窍不得精微濡养,治疗当分脏辨治,予益肺养阴,祛湿化浊,升提宣发以达脾升肺气之清以滋目;疏肝健脾,养血滋阴,升清透郁以达脾升肝气之清以疏目;健脾宁心,养血安神,疏利血脉以达脾升心气之清以养目;健脾补肾,固本生精,升清润目以达脾升肾气之清以润目。运用脾主升清理论辨治干眼,以期丰富中医药治疗干眼的临床思路与方法。Combined with the clinical characteristics of dry eye, assisted by modern clinical application and research, this paper puts forward the principles, methods, prescriptions and medicines for the treatment of dry eye based on the theory of spleen governing and promoting essence Qi. The occurrence of dry eye is mostly caused by the inability of the eyes to be nourished the essence, and the essence and Qi of the five Zang and six Fu is infused into the eyes with the help of spleen which governs and promotes essence Qi. Spleen is the acquired foundation and the origin of Qi and blood. The essence of Qi and blood of the five internal organs is infused into the eyes through the pivot of spleen Qi, so as to nourish the eyes and block the occurrence of dry eye.In the process of preventing and treating dry eye with the theory of spleen governing and promoting essence Qi, if the eyes can’t be nourished by the essence, the therapy should be given based on differentiation of viscera. The lung should be tonified and Yin be nourished to remove dampness as well as turbidity, meanwhile ascend and disperse Qi, so as to achieve the ascending the clear lung Qi promoted by the spleen to nourish the eyes. The liver should be soothed and the spleen be strengthened to nourish the blood and Yin, meanwhile ascend the clear Qi and penetrate depression so as to achieve the ascending of clear liver Qi promoted by the spleen to soothe the eyes. The spleen should be invigorated and the heart be calmed to nourish the blood and tranquilize the mind, meanwhile dredge and benifit the blood vessels, so as to achieve the ascending of cleas heart Qi promoted by the spleen to nourish the eyes. The spleen should be invigorated and the kidney be strengthened to consolidate the constitution and producing essence,meanwhile promote the clear Qi and moistene the eyes so as to achieve the ascending of clear kidney Qi promoted by the spleen to moisten the eyes.
