中医药基于肠道菌群治疗糖尿病视网膜神经退行性病变的研究进展

摘　　要：糖尿病视网膜病变(DR)被认为是一种微血管疾病,早期存在视网膜神经退行性病变,其在DR的发生发展中起到重要作用,而人体肠道微生物及其代谢产物可以抑制视网膜神经退行性病变,这为肠道菌群治疗DR提供了可行性。中医学认为,肾精化髓充脑以养神,肠脑相连,互为属络,故中医药基于肠道菌群治疗DR,可从整体原则出发,平衡全身阴阳,调节代谢水平,再针对局部眼部症状,进行辨证施治。益生菌、肠道代谢物、针刺和中药的应用给中医药防治DR提供了新策略,已在临床上取得了一定的疗效。本文对肠道菌群与视网膜神经退行性病变之间的相互作用进行总结,并综述中医药基于肠道菌群干预防治糖尿病视网膜神经退行性病变及其有关疾病的国内外研究,目的在于展示中医药治疗DR的优势,为未来DR的治疗提供新思路。Diabetic retinopathy(DR)is considered as a microvascular disease,and optic neurodegeneration exists in the early stage,which plays an important role in the occurrence and development of DR.Human intestinal microbes and their metabolites can inhibit optic neurodegeneration,which provides feasibility for the treatment of DR by intestinal flora.In Traditional Chinese Medicine(TCM),it is believed that the kidney formed marrow can nourish the mind,and the intestine and brain are linked,they pertain to and connected with each other.Therefore,TCM can treat DR based on the intestinal flora,starts from the holistic principle,to balance the whole body Yin and Yang,regulate the metabolism level,and then treat the local symptoms of the eye based on syndrome differentiation.The application of probiotics,intestinal metabolites,acupuncture and TCM has provided a new strategy for the prevention and treatment of DR,which has achieved certain clinical effects.In this paper,the interaction between intestinal flora and optic neurodegeneration was summarized,and the domestic and abroad studies on the prevention and treatment of diabetic optic neurodegeneration and related diseases by TCM based on intestinal flora were reviewed,aiming to demonstrate the advantages of TCM in the treatment of DR,and provide new ideas for the future treatment of DR.
