基于名医医案的流感样病例证候与用药的相关性研究

摘　　要：目的挖掘名医治疗流感样病例的辨证用药规律,为临床辨治提供参考。方法收集名医治疗流感样病例的医案,运用“Excel数据挖掘插件和医案大数据分析系统”,对辨证、药物进行频次及关联分析,将结果与传统认知及教科书比较、验证。结果纳入医案200例,涉及15个证型、189味中药,各证相关联的药物大部分与传统理论及教科书一致,能够体现相应的处方和治法,同时有一些新发现,如寒温并用、治疗气分证加解表药、热毒壅肺加僵蚕、滋阴用石斛、热入营血加重楼。结论研究结果反映了名医治疗流感样病例的思想及经验,更体现了临证的整体观、动态观,可为临床提供参考。Objective To explore the law of syndrome differentiation and medication of famous doctors in the treatment of influenza cases,so as to provide reference for clinical treatment.Methods The medical records of famous doctors treating influenza cases were collected,and the“Excel data mining plug-in and medical record big data analysis system”were used to analyze the frequency and association of syndrome differentiation and drugs,and the results were compared and verified with traditional cognition and textbooks.Results 200 cases were included,involving 15 syndrome types and 189 kinds of traditional Chinese medicine.Most of the drugs related to each syndrome were consistent with the traditional theories and textbooks,which could reflect the corresponding prescriptions and therapeutic methods.At the same time,there were some new discoveries,such as the combination of cold and warm,the treatment of Qifen syndrome with antipyretic drugs,heat toxin obstructing lung with stiff silkworm,nourishing yin with dendrobium,and adding paris root for hot blood.Conclusion The results reflect the thinking and experience of famous doctors in treating influenza cases,and also reflect the holistic and dynamic view of clinical symptoms,which can provide reference for clinical practice.
