补中益气汤加减联合盆底康复仪与盆底肌训练在压力性尿失禁康复中的应用

摘　　要：目的分析在盆底功能障碍性疾病康复中补中益气汤加减联合盆底康复仪与盆底肌训练的应用效果。方法将2020年4月至2021年9月在广东医科大学附属惠东医院产科接受治疗的压力性尿失禁患者120例作为研究对象,回顾分析患者相关资料,根据盆底功能康复治疗方案的不同,将其分为两组,每组各60例。对照组予盆底康复仪与盆底肌训练,基于此,观察组增加补中益气汤加减治疗,比较两组盆底肌肌力、尿动力学指标、生活质量评分(PFDI-20评分)及治疗效果。结果观察组肌力分级优于对照组,差异有统计学意义(P<0.05);观察组腹压漏尿点压(ALPP)、最大尿道闭合值(MUCP)、最大尿液率(Qmax)、最大尿认率时逼尿肌压力(Pdet.Qmax)高于对照组,差异有统计学意义(P<0.05)。观察组盆腔脏器脱垂困扰(PDPDI-6)、肛门困扰(CARDI-8)、排尿困扰(UDI-6)评分低于对照组,差异有统计学意义(P<0.05)。观察组治疗效果优于对照组,差异有统计学意义(P<0.05)。结论在压力性尿失禁康复中,采用补中益气汤加减联合盆底康复仪与盆底肌训练治疗,既能改善盆底肌肌力与尿动力学指标,又能改善治疗效果,值得推广。Objective To analyze the application effect of modified Buzhong Yiqi Decoction combined with pelvic floor rehabilitation instrument and pelvic floor muscle training in the rehabilitation of pelvic floor dysfunction diseases.Methods A total of 120 patients with stress urinary incontinence treated in the Department of Obstetrics at the Affiliated Huidong Hospital of Guangdong Medical University from April 2020 to September 2021 were selected as investigation subjects.The patients were retrospected for the relevant data,and were divided into two groups of 60 cases each according to the differences in pelvic floor functional rehabilitation schemes.The control group received treatment with pelvic floor rehabilitation instrument and pelvic floor muscle training,based on this,the observation group was treated with modified Buzhong Yiqi Decoction.The pelvic floor muscle strength,urodynamic index,quality of life score(PFDI-20 score)and effective rate of treatment were compared between the two groups.Results The muscle strength grading of the observation group was better than that of the control group,with a statistically significant difference(P<0.05).The abdominal leak point pressure(ALPP),maximum urethral closure pressure(MUCP),maximum urinary flow rate(Qmax),and detrusor pressure at the maximum urinary flow rate(Pdet.Qmax)in the observation group were higher than those in the control group,with statistically significant differences(P<0.05).The pelvic organ prolapse distress(PDPDI-6),anal distress(CARDI-8),and urination distress(UDI-6)scores in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).The effective of treatment in the observation group was better than that in the control group,with a statistically significant difference(P<0.05).Conclusion In the rehabilitation of stress urinary incontinence,the application of modified Buzhong Yiqi Decoction combined with pelvic floor rehabilitation instrument and pelvic floor muscle training can not only improve
