摘　　要：目的探讨当归活血液内服联合三黄膏外敷治疗桡骨远端骨折的临床价值。方法选取2019年9月—2020年12月于长沙市中医医院南院骨伤科门诊及住院部行中医手法复位夹板外固定治疗的桡骨远端骨折病患64例,按随机数字表法分为对照组和观察组,每组32例。对照组给予手法复位小夹板外固定治疗,观察组在对照组基础上加用当归活血液内服与三黄膏外敷治疗。对比治疗前后2组肿胀指数、疼痛程度及血清炎症因子表达水平。结果2组患者肿胀指数术前差异无统计学意义(P>0.05),在术后第4天、第7天、第14天,观察组较对照组肿胀指数改善的程度均更明显(P<0.05);2组患者VAS评分术前差异无统计学意义(P>0.05),术后第4天、第7天、第14天观察组较对照组效果更优(P<0.05);2组患者血清中CRP、IL-1β、IL-6的含量术前差异均无统计学意义(P>0.05),术后第14天观察组较对照组下降效果更显著(P<0.05)。结论当归活血液内服联合三黄膏外敷治疗桡骨远端骨折具有应用价值,效果显著,能够起到缓解关节疼痛、促进肿胀消退、降低血清中炎症因子表达水平等作用。Objective To investigate the clinical value of Danggui Huoxue decoction combined with Sanhuang ointment in the treatment of distal radius fracture.Methods From September 2019 to December 2020,64 patients with distal radius fracture treated with manual reduction and splint external fixation were randomly divided into control group and observation group,with 32 cases in each group.Two groups of patients were given manual reduction,small splint external fixation treatment,and the observation group was given Danggui Huoxue decoction and external application of Sanhuang ointment on this basis.The swelling index,pain degree and serum cytokine expression were compared before and after treatment.Results There was no significant difference in the swelling index between the two groups before surgery(P>0.05).On the 4th,7th and 14th day after surgery,the swelling index of the observation group was improved more significantly than that of the control group(P<0.05).The VAS scores of the 2 groups had no statistical significance before surgery(P>0.05),and the effect of the observation group was better than that of the control group on the 4th,7th and 14th days after surgery(P<0.05).The levels of CRP,IL-1βand IL-6 in serum of the two groups had no significant difference before operation(P>0.05),and the effect of the observation group was more significant than that of the control group on the 14th day after operation(P<0.05).Conclusion Danggui Huoxue decoction combined with Sanhuang ointment in the treatment of distal radius fracture has application value,significant effect,can alleviate joint pain,promote swelling regression,and reduce the expression of inflammatory factors in serum and other effects.
