摘　　要：目的 评估艾司氯胺酮对剖宫产术后镇痛效果和产后抑郁的影响及其不良反应。方法 选取2020年10月~2021年4月在某院产科行剖宫产的产妇共70例,其中对照组40例,试验组30例。两组产妇术后均使用自控静脉镇痛泵,对照组镇痛泵中的药物为舒芬太尼100μg,而试验组为舒芬太尼50μg加艾司氯胺酮50 mg。采用视觉模拟评分法(VAS)评价术后12,24,48 h 3个时间点的疼痛程度,并记录使用镇痛泵的不良反应发生情况。术后1周根据爱丁堡产后抑郁量表(EPDS)评价产妇抑郁情况。结果 两组的VAS评分、EPDS评分以及不良反应发生率的差异均无统计学意义(P>0.05)。结论 艾司氯胺酮用于剖宫产术后镇痛安全有效,但本研究未发现它对产后抑郁有预防作用。OBJECTIVE To observe the analgesic effect,antidepressant effect and adverse reaction of esketamine in puerperant after cesarean section.METHODS 70 parturients undergoing cesarean section in a hospital during October 2020 to April 2021 were randomly divided into two groups,including 40 cases in the control group and 30 cases in the experimental group.The study subjects in both groups used the patient-con trolled intravenous analgesic pump after operation.Analgesics in the pump were sufentanil 100 μg in the control group,and sufentanil 50 μg plus esketamine 50 mg in the experimental group.VAS was used to evaluate the degree of pain at three time points including 12 hours,24 hours,and 48 hours after surgery,and the occurrence of adverse reactions was recorded.Maternal depression was evaluated 1 week after surgery according to EPDS.RESULTS There was no significant difference(P>0.05) between the two groups in VAS score,EPDS score and adverse reactions.CONCLUSION Esketamine is safe for postoperative analgesia after cesarean section,with proven analgesic effect.Therefore,its effect on the prevention of postpartum depression is not observed.
