摘　　要：目的探讨对子宫瘢痕妊娠(CSP)患者清宫术前应用米非司酮、甲氨蝶呤联合治疗效果及对β-人绒毛膜促性腺激素(β-HCG)水平的影响。方法选择2016—2019年收治的CSP 129例,根据治疗方案分为对照组67例和观察组62例。对照组给予甲氨蝶呤治疗,观察组在对照组基础上给予米非司酮治疗。比较2组治疗效果、围术期指标及治疗前后β-HCG水平,观察安全性。结果观察组总有效率高于对照组,出血持续时间、包块吸收时间、住院天数均短于对照组,血β-HCG水平低于对照组(P<0.05,P<0.01)。2组不良反应发生率比较差异无统计学意义(P>0.05)。结论CSP患者清宫术前应用甲氨蝶呤联合米非司酮治疗安全有效,能降低β-HCG水平,缩短恢复时间。Objective To investigate the effect of Mifepristone combined with Methotrexate on patients with cesarean scar pregnancy(CSP)before uterine curettage and its effect onβ-human chorionic gonadotropin(β-HCG)levels.Methods A total of 129 patients with CSP treated from 2016 to 2019 were selected and divided into the control group(n=67)and the observation group(n=62)according to the treatment scheme.The control group was treated with Methotrexate,and the observation group was treated with Mifepristone on the basis of the control group.The therapeutic effects,perioperative indexes andβ-HCG level before and after treatment were compared between the two groups,and the safety was observed.Results The total effective rate of the observation group was higher than that of the control group.The duration of bleeding,mass absorption time and length of hospital stay were shorter than those of the control group,and theβ-HCG level was lower than that in the control group(P<0.05,P<0.01).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Methotrexate combined with Mifepristone is safe and effective in the treatment of CSP patients before uterine curettage,which can lower theβ-HCG level and shorten recovery time.
