摘 要:目的探讨高分辨率磁共振成像(HRGMRI)在颅内动脉斑块的临床应用价值,比较多种MR扫描技术对有临床症状脑卒中患者的动脉斑块及其供血区域脑卒中的辨别分析.方法回顾性分析21例有缺血性脑卒中症状的颅内动脉粥样硬化患者的影像资料及临床指标,所有患者均行HRGMRI、磁共振血管成像(MRA)、扩散加权成像(DWI)及常规MR序列成像,对比多种扫描序列对颅内动脉斑块及脑卒中的检出优势.结果(1)三维容积T1WI抑脂(3DGT1 CubeFS)序列对具有动脉斑块的血管检出较DWI和MRA对相应供血范围病变的检出更多(χ^(2)=0.420、9.977,P<0.001、0.002);(2)左椎动脉(VA)斑块数目比基底动脉(BA)、右VA、左大脑中动脉(MCA)、右MCA、左大脑后动脉(PCA)、右PCA检出多(P<0.001);(3)易损斑块分布区发生急性脑梗死可能性更大(χ^(2)=31.508,P<0.001).结论颅内HRGMRI序列对缺血性脑卒中的检出和预测比DWI和MRA更有优势.颅内血管斑块主要分布在大血管,即VA及颈内动脉(ICA),其次是BA及MCA.Objective To evaluate the clinical application value of high resolution magnetic resonance imaging(HR-MRI)in intracranial arterial plaques,and to compare the differences of multi-MR sequences in detecting cerebral arterial plaque and the region of blood supply in the patients with clinical symptoms of stroke.Methods The imaging and clinical data of 21 patients diagnosed as intracranial atherosclerosis with symptoms of ischemic stroke were analyzed ret rospectively.All patients underwent HR-MRI,magnetic resonance angiography(MRA),dffusionou weighted imaging(DWI)and conventional MR sequence.The advantages of multiple scanning sequences in detecting intacranial arterial plaque and stroke were compared.Results(1)Three dimensional Cube T,W1 fat suppression(3D-T_(1) Cube FS)sequence could deteet more vessels with plaques than DWI or MRA with lesions corresponding blood supply range(χ^(2)=0.420,9.977,P<0.001,0.002).(2)The plaques on left vertebral artery(VA)were more than those on basilar artery(BA),right VA,left and right middle cerebral artery(MCA),left and right posterior cerebral artery(PCA)(P<0.01).(3)The probability of acute cerebral infarction was higher in the vulnerable plaque distribution(χ^(2)=31.508,P<0.001).Conclusion Intracranial H R-MRI in displaying intracranial vessel wall is more advantagecous in detecting or predicting acute ischemic stroke than DWI and MRA;intracranial vascular plaques are mainly distibuted in the greater vessels as VA and internal carotid artery(ICA),flloweld by BA and MCA.
