青年女性乳腺癌X线征象与病理特征对照研究

摘　　要：目的探讨数字化乳腺X线摄影对青年女性乳腺癌的诊断价值.方法收集经病理证实的76例青年女性乳腺癌患者临床及X线影像资料,乳腺影像和数据报告系统(BIGRADS)4B类以上为检出阳性,回顾性分析X线征象与病理、患者年龄与乳腺癌分期、免疫组化指标之间的关系.结果76例乳腺癌中,年龄<30岁17例,30~35岁59例,X线摄影检出阳性60例,占78.95%,并且以肿块和钙化为主要表现(54例).临床分期0~Ⅰ期25例,ⅡA期28例,ⅡB期12例,Ⅲ期9例,Ⅳ期2例,病理结果以浸润性导管癌为主,免疫组化提示雌激素受体(ER)、孕激素受体(PR)、人表皮生长因子受体2(HERG2)阳性率分别为67.11%、76.32%和76.32%,并且在30岁以下年龄组较高(70.58%、82.35%和82.35%).结论乳腺X线摄影检查青年女性乳腺癌的阳性率高,有助于青年女性乳腺癌的早期诊断.Objective To explore the diagnostic value of digital mammography in young women with breast cancer.Methods The clinical and mammography data of 76 young women patients pathologically confirmed as breast cancer were collected and analyzed.Breast imaging reporting and data syster(BI-RADS)4B and above were considered positive cases.The relationships between X-ray signs and pathology,age and breast cancer stage,and immune-histochemical results were analyzed respectively.Results There were 17 patientsV30 years old and 59 patients 30-35 years old.A total of 60 out of 76 young women patients with breast cancer(78.95%)were positive in mammography.Among these cases,mass and calcification(54 patients)were mainly showed on digital mammographic images in the present study.There were 25 cases of stage 0-I,28 cases of stageⅡ A,12 cases of stage Ⅱ B,9 cases of stageⅢ,and 2 cases of stage Ⅳ.The mainly pathological result was the infiltrating ductal carcinoma.The results of immuno-histochemical showed the positive rates of estrogen receptor(ER),progesterone receptor(PR),and human epidermal growth factor receptor 2(HER-2)were 67.11%,76.32%and 76.32%,respectively,which were higher in the group under 30 years old(70.58%,82.35%and 82.35%,respectively).Conclusion Mammography has a good performance in detecting breast cancer in young women and can be used as an early screening method.
