摘　　要：目的:探究小剂量低分子肝素钙与双嘧达莫联合醋酸泼尼松治疗肾病综合征(NS)患儿的效果。方法:选取94例NS患儿,按照随机数字表法分为观察组和对照组,每组各47例。对照组给予双嘧达莫联合醋酸泼尼松治疗,观察组给予小剂量低分子肝素钙联合醋酸泼尼松治疗。对比两组临床疗效、肾功能指标[尿素氮(BUN)、血肌酐(SCr)、24 h尿蛋白定量(24 h Upro)及血肌酐清除率(Ccr)]、凝血功能指标[部分活化凝血活酶时间(APTT)、抗凝血酶原Ⅲ(AT-Ⅲ)、凝血酶原时间(PT)、D-二聚体、纤维蛋白原(Fb)、血小板]、白蛋白、总胆固醇(TC)水平。结果:观察组总有效率高于对照组(P<0.05);治疗后,观察组BUN、SCr、24 h Upro、Fb、D-二聚体、血小板、TC水平均低于对照组,Ccr、AT-Ⅲ、白蛋白均高于对照组,APTT、PT均长于对照组(P<0.05)。结论:小剂量低分子肝素钙联合醋酸泼尼松可有效缓解临床症状,改善肾功能及凝血功能。Objective:To explore the effects of low-dose low molecular weight heparin calcium and dipyridamole combined with prednisone acetate tablets in the treatment of children with nephrotic syndrome(NS).Methods:94 cases of NS children were randomized into the observation group and control group,each with 47 cases.The control group were treated with dipyridamole combined plus prednisone acetate,while the observation group were given low dose of low molecular weight heparin calcium combined with prednisone acetate.The clinical efficacy,renal function indexes[urea nitrogen(BUN),serum creatinine(SCr),24 h Upro and serum creatinine clearance rate(Ccr)],coagulation function indicators[activated partial thromboplastin time(APTT),antithrombinⅢ(AT-Ⅲ),prothrombin time(PT),D-dimer,fibrinogen(Fb),and platelet],albumin,and total cholesterol(TC)levels were compared between both groups.Results:The total effective rate of the observation group was higher than that of control group(P<0.05);after treatment,the levels of BUN,SCr,24 h Upro,Fb,D-dimer,platelet and TC in observation group were lower than those in control group;Ccr,AT-Ⅲand albumin were higher than those in the control group,and APTT and PT were longer than those in control group(P<0.05).Conclusion:For NS patients,low dose of low molecular weight heparin calcium combined with prednisone acetate can effectively relieve their clinical symptoms,and improve renal function and coagulation function.
