摘　　要：基本医疗保险作为一项治理贫困、促进社会公平正义的重要制度,对于巩固脱贫攻坚成果具有重要意义。基于2018年CFPS数据,采用IV-Probit模型并控制模型存在的内生性问题,探究基本医疗保险对城乡居民的扶贫绩效。结果发现:(1)基本医疗保险对城乡居民扶贫绩效显著。(2)基本医疗保险扶贫绩效存在异质性,对农村、西部地区、老年人等相对弱势群体扶贫绩效更高;然而,基本医疗保险保障水平相对较低,对于重大疾病冲击产生的高额医疗费用,基本医疗保险扶贫绩效有限。因此,应发挥基本医疗保险在防止因病致贫、因病返贫中的作用,同时逐步提高基本医疗保险保障水平和保障范围,对老年人等弱势群体适当倾斜医疗资源,促进社会公平。As an important system of poverty control and social equity and justice, social medical insurance is of great significance for consolidating the achievements of poverty alleviation. Based on 2018 CFPS data, this paper uses the IV-Probit model to control the endogeneity problem existing in the model and explores the poverty alleviation performance of social medical insurance for urban and rural residents. The results showed that:(1) Social medical insurance has a significant effect on poverty alleviation among urban and rural residents;(2) The heterogeneity of poverty alleviation performance of social medical insurance was explored according to the three dimensions of urban and rural areas, regions and ages. The poverty alleviation performance of relatively vulnerable groups such as rural areas, western regions, and the elderly was higher. However, the guarantee level of social medical insurance is relatively low, and the poverty alleviation performance of social medical insurance is limited to the high medical expenses caused by the impact of major diseases. This paper suggests that we should continue to play the role of social medical insurance in preventing poverty caused by illness and returning to poverty due to illness, and at the same time, we should gradually improve the level and scope of social medical insurance, appropriately prioritize medical resources for the elderly and other vulnerable groups, promote social equity and justice.
