摘 要:目的构建抗菌药物管控的新模式,探索形成长效的管理机制。方法将六西格玛管理的定义、测量、分析、改进和控制五步法应用到AMS模式,识别管控最关键的影响因素,并针对性采取改进措施。比较2018年至2021年9月六西格玛应用前后的相关评价指标。结果应用管控新模式后,抗菌药物合理使用率、微生物标本送检率逐年提升,住院患者人均药费及抗菌药费显著降低,而平均住院天数差异无统计学意义。结论AMS模式下的六西格玛管理模式可促进临床抗菌药物合理使用,相关经验可为各医疗机构提供参考。OBJECTIVE To explore the long-term mechanism of antibacterial drug use by establishing a new management model.METHODS Five phases of Six-Sigma(define,measure,analyze,improve and control)were used to find out the key factors that affected AMS,targeted improvement measures were taken.The data from 2018 to 2021 September were analyzed before and after adopting Six-Sigma was compared.RESULTS After applying management model for antibacterial drug use,the rational use rate of antibiotics and the submission rate of microbial samples increased year by year;the per capita drug cost and antibacterial drug cost of inpatients decreased significantly,but the difference in average hospitalization days was not statistically significant.CONCLUSION Six Sigma management model under AMS model can promote rational use of antibiotics,and relevant experience can provide reference for medical institutions.
