摘　　要：目的了解某院碳青霉烯类抗菌药物的临床使用情况,下一步规范该类药品管理提供参考依据。方法建立符合某院实际的碳青霉烯类抗菌药物利用评价标准,并以此对2019年第一季度使用碳青霉烯类抗菌药物的100份出院病历利用topsis法进行优劣评价。结果100份病例中,总Ci平均值为0.7214,用药基本合理。其中不合理病历(Ci<0.6)的病历占11%。按科室分层,使用前3位的分别是呼吸内科、泌尿外科和危重医学科,占76%,不合理病历分别占8.89%、10.3%、16.67%。结论基于加权TOPSIS法的碳青霉烯类抗菌药物利用评价方法是可行的,评价结果直观、全面,可为建立碳青霉烯类药物一体化管理体系奠定基础。OBJECTIVE To understand the clinical use of carbapenems in a hospital,and provide reference for implements standardization management of carbapenems in the following steps.METHODS Establish the carbapenems utilization evaluation criteria according to the actual situation of the hospital,and the TOPSIS method was used to evaluate the advantages and disadvantages of 100 discharged medical records using carbapenems in the first quarter of 2019.RESULTS The average Ci of these records was 0.7214,indicating that the drug use was reasonable.The unreasonable medical records(Ci<0.6)accounted for 11%.The top three departments were respiratory medicine,urology and critical care medicine,accounting for 76%,and unreasonable medical records account for 8.89%,10.3% and 16.67%,respectively.CONCLUSION The drug utilization evaluation method of carbapenems based on weighted TOPSIS method is feasible,and the evaluation results are intuitive and comprehensive,which can lay a foundation for the establishment of carbapenems integrated management system.
