经阴道彩超在卵巢癌早期诊断中的应用价值分析

摘　　要：目的分析经阴道彩色多普勒超声(彩超)早期诊断卵巢癌的应用效果。方法100例疑似早期卵巢癌患者,均给予腹部彩超检查、经阴道彩超检查进行诊断。以手术病理结果作为金标准,比较两种检查方法的阳性预测值、阴性预测值、灵敏度与特异度。结果手术病理结果显示,100例患者中卵巢癌50例,良性肿瘤50例。经阴道彩超检查对卵巢癌的阳性预测值80.70%(46/57)、阴性预测值90.70%(39/43)、灵敏度92.00%(46/50)与特异度78.00%(39/50)均高于腹部彩超检查的63.33%(38/60)、70.00%(28/40)、76.00%(38/50)、56.00%(28/50),差异具有统计学意义(χ^(2)=4.355、5.704、4.762、5.473,P<0.05)。结论经阴道彩超在卵巢癌早期诊断中应用价值显著,可获得较高的灵敏度、特异度,有助于临床病情的及时确诊、有效治疗。Objective To analyze the application effect of transvaginal color Doppler ultrasound in early diagnosis of ovarian cancer.Methods 100 cases of suspected early ovarian cancer patients were diagnosed by abdominal color Doppler ultrasound and transvaginal color Doppler ultrasound.Using surgical and pathological results as the gold standard,the positive predictive value,negative predictive value,sensitivity and specificity of the two methods were compared.Results The results of surgery and pathology showed that among the 100 patients,there were 50 cases of ovarian cancer and 50 cases of benign tumors.The positive predictive value of transvaginal color Doppler ultrasound for ovarian cancer was 80.70%(46/57),the negative predictive value was 90.70%(39/43),the sensitivity was 92.00%(46/50)and the specificity was 78.00%(39/50),which were higher than 63.33%(38/60),70.00%(28/40),76.00%(38/50),and 56.00%(28/50)of abdominal color Doppler ultrasound,and the differences were statistically significant(χ^(2)=4.355,5.704,4.762,5.473;P<0.05).Conclusion Transvaginal ultrasound is of significant value in the early diagnosis of ovarian cancer,and can obtain high sensitivity and specificity,which is helpful for the timely diagnosis and effective treatment of clinical conditions.
