摘  要:目的:分析新生儿三尖瓣环收缩期位移(TAPSE)和右心室面积与胎龄的相关性。方法:选取2017年1月—2019年9月中山市博爱医院新生儿科350例1~3 d新生儿,其中,早产儿122例,足月产儿167例,过期产儿61例。分别对不同胎龄组的新生儿进行测量T A P SE、舒张期末右室面积(RV E DA)和收缩期末右室面积(RV E SA),并计算右室面积变化分数(FAC)值。结果:新生儿的TAPSE、RVEDA、RVESA随着胎龄的增长而增大,组间对比差异均有统计学差异(P<0.01);组间的FAC对比,差异无统计学意义(P>0.05);新生儿的TAPSE、RVEDA及RVESA与胎龄均呈显著相关性(P<0.01);但新生儿的FAC与胎龄无显著相关性(P>0.05)。新生儿的TAPSE、RVEDA及RVESA与新生儿HR无相关性(P>0.05);新生儿的TAPSE与RVEDA、RVESA均呈显著相关性(P<0.01);但新生儿的FAC与TAPSE无相关性(P>0.05)。结论:TAPSE能更好地评估新生儿的右室收缩功能,且不受新生儿心率的影响,有助于指导治疗及判断预后。Objective To investigate the value of tricuspid annular systolic displacement(TAPSE)in the evaluation of neonatal right cardiac structure and function.Methods 350 newborns of 1~3 days in the Department of Neonatology of Zhongshan Bo’ai hospital from January 2017 to September 2019 were randomly selected as the research objects,including 122 premature infants,167 full-term infants and 61 overdue infants.TAPSE,end diastolic right ventricular area(rveda)and end systolic right ventricular area(rvesa)were measured in newborns of different gestational age groups,and the change fraction of right ventricular area(FAC)was calculated.Results TAPSE,rveda and rvesa of newborns increased with the increase of gestational age.There were significant statistical differences between the groups(P<0.01);There was no significant difference in fac between groups(P>0.05);TAPSE,rveda and rvesa of newborns were significantly correlated with gestational age(P<0.01);However,there was no significant correlation between neonatal fac and gestational age(P>0.05).There was no correlation between neonatal TAPSE,rveda and rvesa and neonatal HR(P>0.05);There was significant correlation between TAPSE,rveda and rvesa in newborns(P<0.01);However,there was no correlation between neonatal fac and TAPSE(P>0.05).Conclusion TAPSE can have better evaluate the right ventricular systolic function of newborns,and is not affected by neonatal heart rate.It is helpful to guide treatment and judge prognosis.
