摘　　要：目的:探讨在椎基底动脉供血不足性眩晕(VBIV)患者中施以自拟平肝熄风和血汤联合穴位贴敷治疗的临床效果。方法:纳入鹰潭市妇幼保健院2018年4月至2021年8月收治的84例VBIV患者,采用随机数字表法分为对照组和观察组,各42例。对照组予以常规西医治疗,观察组加用自拟平肝熄风和血汤联合穴位贴敷治疗,均连续治疗2周。对两组临床疗效、椎基底动脉平均血流速度(Vm)、血液流变学指标及不良反应进行比较。结果:观察组临床总有效率及右椎动脉(RVA)、左椎动脉(LVA)、基底动脉(BA)的Vm均高于对照组,全血高切黏度(HBV)、血浆黏度(PV)和全血黏度(LBV)水平均低于对照组,均差异有统计学意义(均P<0.05);两组治疗期间均未见明显不良反应。结论:在VBIV患者中应用自拟平肝熄风和血汤联合穴位贴敷治疗,可提高椎基底动脉血流速度,促进血液循环,增强临床疗效,是一种安全、有效的治疗方案,适宜临床推广。Objective:To investigate the clinical effect of self-prepared pinggan xifeng hexue decoction combined with acupoint application in patients with vertebrobasilar artery insufficiency vertigo(VBIV).Methods:A total of 84 VBIV patients admitted to Yingtan Maternity and Child Health Hospital from April 2018 to August 2021 were enrolled.They were divided into control group and observation group by random number table method,with 42 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with selfmade pinggan xifeng hexue decoction combined with acupoint application,and the treatment was continued for 2 weeks.The clinical efficacy,mean blood flow velocity(Vm)of the vertebrobasilar artery,hemorheology indexes and adverse reactions were compared between the two groups.Results:The total clinical effective rate and the Vm of the right vertebral artery(RVA),left vertebral artery(LVA)and basilar artery(BA)of the observation group were higher than those of the control group.Whole blood high shear viscosity(HBV),plasma viscosity(PV)and the level of whole blood viscosity(LBV)was lower than that of the control group,and there was a statistical difference(P<0.05);there were no obvious adverse reactions during the treatment period of the two groups.Conclusion:The application of selfmade pinggan xifeng hexue decoction combined with acupoint application in VBIV patients can increase the vertebrobasilar artery blood flow velocity,promote blood circulation,and enhance clinical efficacy.It is a safe and effective treatment plan,suitable for clinical use promotion.
