摘 要:目的了解阴道微生态与宫颈人乳头瘤病毒(human papillomavirus,HPV)感染的相关性。方法选取2020-06/2020-12月在某三甲妇幼保健机构健康中心行常规妇科体检的680名妇女,均行阴道微生态、宫颈HPV检测。根据HPV感染情况分为HPV阳性组(n=121),HPV阴性组(n=559)。结果HPV阳性组16、18型高危阳性有33人(27.27%),非16、18型的高危阳性有75人(61.98%),仅低危HPV感染的有13人(10.74%)。HPV阳性组51例(42.15%)合并多重HPV感染。本研究中人群阴道微生态失衡的有342例(50.19%),其中细菌性阴道炎(bacterialvag-initis,BV)最为常见,其次为外阴阴道假丝酵母菌病(vulvovaginal candidiasis,VVC),其中混合性阴道炎、pH值与HPV感染差异具有统计学意义(P<0.05)。Logistic回归分析显示混合性阴道炎是HPV感染的独立危险因素(OR=2.479,95%CI:1.403~4.382)。结论临床上针对HPV感染,应采用改善阴道微环境,维持正常菌群的方案。治疗混合性阴道炎,对于降低HPV感染率,提高HPV的清除率起着重要的作用。Objective To investigate the correlation between vaginal microecology and cervical human papillomavirus(HPV)infection.Methods A total of 680 women who underwent gynecological physical examination in a level 3 maternal and child health care institution were selected from June to December 2020,and they were tested in vaginal microecology and cervical HPV.According to condition of HPV infection,the patients were divided into HPV positive group(n=121)and HPV negative group(n=559).Results In HPV positive group,33 cases(27.27%)were high-risk positive of 16 and 18 types,75 cases(61.98%)were high-risk positive for non 16 and 18 type,and 13 cases(10.74%)were low-risk HPV infection.51 cases(42.15%)in HPV positive group complicated with multiple HPV infection.There were 342 cases(50.19%)of vaginal microecological imbalance,which bacterial vaginitis(BV)was the most common,the second was vulvovaginal candidiasis(VVC).There was statistical significance between mixed infection,pH and HPV infection(P<0.05).Logistic regression showed that mixed vaginitis was an independent risk factor for HPV infection(OR=2.479,95%CI:1.403-4.382).Conclusion It is necessary to improve vaginal microenvironment,maintain normal flora.Treatment of mixed infections plays an important role in reducing HPV infection and improving HPV clearance.
