摘 要:目的探讨肌骨超声(musculoskeletal ultrasound,MSUS)诊断急性闭合性跟腱断裂训练伤的应用价值。方法回顾性分析2017-03/2021-06月作者医院收治的经手术证实急性闭合性单侧跟腱断裂训练伤的伤员87例,比较MSUS与磁共振成像(magnetic resonance imaging,MRI)诊断急性闭合性跟腱断裂训练伤的临床价值。结果手术证实完全型跟腱断裂的77例,其中MSUS诊断完全型72例,部分型5例;MRI诊断完全型76例,部分型1例。手术证实部分型跟腱断裂的10例,MSUS诊断部分型9例,完全型1例;MRI诊断部分型10例。MSUS与MRI的诊断跟腱断裂的一致性好,Kappa值为0.767(P<0.001)。MSUS及MRI对跟腱断裂的诊断敏感性均为100%。对其分型诊断敏感性相近,均高达90%以上。结论相对于MRI而言,MSUS更适应军事训练或战场条件,因此在军事训练伤中具有较高的军事医学应用价值。Objective To investigate the application value of musculoskeletal ultrasound(MSUS)in closed acute achilles tendon rupture of military training injury.Methods Retrospective analysis was conducted on 87 patients who underwent unilateral surgical treatment of closed acute achilles tendon rupture from March 2017 to June 2021 in author′s hospital.The clinical value of MSUS and magnetic resonance imaging(MRI)in the diagnosis of closed acute achilles tendon rupture was compared.Results Among the 77 patients with complete rupture proved by surgery,72 cases were diagnosed as complete rupture and 5 cases as partial rupture by MSUS;76 cases were diagnosed as complete rupture and 1 case as partial ruptur by MRI.Among the 10 patients with partial rupture proved by surgery,9 cases were diagnosed as partial rupture and 1 case as complete rupture by MSUS;10 cases were diagnosed as partial rupture by MRI.MSUS and MRI had good consistency in diagnosis,and the Kappa value was 0.767(P<0.001).The sensitivity of MSUS and MRI for the diagnosis of achilles tendon rupture was 100%.The sensitivity of its classification was similar,which was above 90%.Conclusion Compared to MRI,MSUS is more suitable for military training or battlefield conditions,so it has a higher military medical application value in military training injury.
