自拟通腑汤治疗脓毒症患者临床观察

摘　　要：目的 探讨自拟通腑汤对脓毒症患者临床疗效及其作用机制探讨。方法 选取脓毒症患者86例,随机分为两组各43例。对照组予西医规范治疗,观察组在此基础上加服自拟通腑汤。比较两组中医证候积分、急性生理及慢性健康状况评分(APACHEⅡ)、5级肠功能衰竭评分(GIF)、全身性感染相关性器官功能衰竭评分(SOFA)、免疫功能、炎性指标变化情况。结果 治疗后两组中医证候积分较前显著降低(P<0.05),且观察组显著优于对照组(P<0.05);两组APACHEⅡ、GIF、SOFA评分均较治疗前明显改善(P<0.05),且观察组较对照组改善显著(P<0.05);观察组免疫功能各指标均显著改善,与对照组治疗后比较差异有统计学意义(P<0.05);观察组炎性因子含量显著低于对照组(P<0.05)。结论 自拟通腑汤可有效改善脓毒症患者中医证候积分,缓解临床症状,降低APACHEⅡ、GIF、SOFA评分,这可能与其降低血清炎性因子及免疫状态有关。Objective:To explore the clinical efficacy and mechanism of Tongfu Decoction on patients with sepsis. Methods:A total of 86 patients with sepsis were selected and divided into two groups according to the treatment methods,43 cases in each group. The control group was treated with western medicine,and the observation group was treated with Tongfu Decoction on the basis of the control group. The TCM symptom score,APACHEⅡscore,Gastrointestinal Failure score(GIF),Sepsis-related Organ Failure Assessment(SOFA),immune function and inflammatory index changes were compared between the two groups. Results:After treatment,the TCM syndrome scores of the two groups were significantly lower than before treatment(P< 0.05),and the observation group was significantly better than that of the control group(P< 0.05);the APACHEⅡ,GIF,SOFA scores of the two groups were significantly improved compared with before the treatment(P< 0.05),and the observation group was significantly improved compared with the control group(P< 0.05);the immune function indexes of the observation group were significantly improved than the control group(P< 0.05);the content of inflammatory factors in the observation group was significantly lower than that of the control group(P< 0.05). Conclusion:Self-designed Tongfu Decoction can effectively improve the TCM syndrome score of patients with sepsis,alleviate clinical symptoms and reduce APACHEⅡ,GIF and SOFA scores,which may be related to the reduction of serum inflammatory factors and immune status.
