丹芍凉血方熏洗结合喜辽妥外用治疗急性期下肢血栓性浅静脉炎的疗效观察

摘　　要：目的探讨丹芍凉血方熏洗结合喜辽妥外用治疗急性期下肢血栓性浅静脉炎(SVT)的疗效及对血液流变学的影响。方法将108例下肢SVT患者随机分为两组,对照组54例患者给予喜辽妥外用,观察组54例患者采用丹芍凉血方熏洗后外用喜辽妥。对比两组的临床疗效与用药不良反应,治疗前后评价主要症状体征评分、患侧与健侧下肢周径差值,检测血液流变学指标全血黏度、血浆黏度和纤维蛋白原(FIB)。结果观察组总有效率为98.15%,显著高于对照组的83.33%(P<0.05)。治疗后,两组主要症状体征评分、患侧与健侧下肢周径差值均显著降低,且观察组显著低于对照组(P<0.05)。治疗后,两组全血高切黏度、全血低切黏度、血浆黏度和FIB均显著降低,且观察组显著低于对照组(P<0.05)。结论丹芍凉血方熏洗结合喜辽妥外用治疗急性期下肢SVT安全有效,可促进临床症状体征消退并改善血液流变学异常,提高治疗效果。Objective:To investigate the effect of Danshao Liangxue Decoction fumigation and washing combined with hirudoid topical treatment for acute superficial vein thrombophlebitis(SVT)of lower extremity and its influence on hemorheology.Methods:A total of 108 patients with SVT of the lower extremity were randomly divided into two groups;54 cases in the control group received hirudoid topical treatment and other conventional western medicine treatment,another 54 cases in the observation group received Danshao Liangxue Decoction fumigation and washing combined with hirudoid topical treatment.The clinical efficacy and adverse reactions of the two groups were compared,the main symptoms and signs scores and the difference value between the affected side and the uninfected side of the lower limbs were evaluated before and after treatment,the hemorheological indicators of whole blood viscosity,plasma viscosity and fibrinogen(FIB)were measured.Results:Compared with the control group,the total effective rate of the observation group was significantly higher(98.15%vs 83.33%,P<0.05).After treatment,the scores of main symptoms and signs,and the difference value between the affected side and the uninfected side of the lower limbs of the two groups were significantly reduced(P<0.05),and the degree of reduction in observation group was more pronounced than that of the control group(P<0.05).After treatment,the average of the whole blood high-shear viscosity,whole blood low-shear viscosity,plasma viscosity and FIB of the two groups were significantly reduced,and the degree of reduction in the observation group were significantly lower than those of the control group(P<0.05).Conclusion:Danshao Liangxue Decoction fumigation and washing combined with hirudoid topical treatment is safe and effective for patients with acute SVT of lower extremity,which can promote the regression of clinical symptoms and signs,improve blood rheology abnormalities,and improve the therapeutic effect.
