摘 要:为探讨冠心病心力衰竭患者PCI术前后心功能及动态心电图的变化及意义,选取常熟市第二人民医院心内科确诊为冠脉病变合并心力衰竭并完成PCI治疗的患者共60例。按照是否存在心血管事件将所有病患分成预后良好组和预后不良组,分别为51例和9例。分析两组患者PCI术前术后的心功能分级以及动态心电图。结果表明,治疗后预后良好组心功能显著优于预后不良组(Z=-2.762,P=0.006)。预后良好组SDNN,SDNN-Index,RMSSD,PNN50的均值均大于预后不良组,比较均有显著统计学差异性(P均<0.05);两组患者的三角指数均值比较无显著统计学差异性(P均>0.05)。60例患者在治疗后,严重心率失常发生率、频发室性早搏率、多源室性早搏发生率、室早RonT率、短阵室性心动过速率均更低(P均<0.05)。动态心电图检查可以监测冠心病心衰患者PCI术后心律失常,是通过无创方法评估PCI术后患者病情及预后的重要指标。In order to explore the changes and significance of cardiac function and ambulatory electrocardiogram in patients with coronary heart disease and heart failure before and after PCI,60 patients diagnosed as coronary artery disease complicated with heart failure and completed PCI in the Department of Cardiology of Changshu Second People’s hospital were selected.According to whether there were cardiovascular events,all patients were divided into good prognosis group and poor prognosis group,51 cases and 9 cases respectively.The cardiac function grading and dynamic electrocardiogram before and after PCI were analyzed.The results showed that the cardiac function of the group with good prognosis was significantly better than that of the group with poor prognosis(Z=-2.762,P=0.006).The mean values of SDNN,SDNN-Index,RMSSD and PNN50 in the good prognosis group were significantly higher than those in the poor prognosis group(P<0.05);there was no significant difference in the mean value of trigonometric index between the two groups(P>0.05).After treatment,the incidence of severe arrhythmia,frequent ventricular premature beats,multi-source ventricular premature beats,ventricular premature ront rate and short array ventricular tachycardia rate were lower in 60 patients(all P<0.05).Ambulatory electrocardiogram can monitor arrhythmia after PCI in patients with coronary heart disease and heart failure.It is an important index to evaluate the condition and prognosis of patients after PCI by noninvasive method.
