健康中国战略下肿瘤康复药学服务模式的探索

摘　　要：目的:探索在健康中国战略下临床药师如何发挥专业优势紧密协同康复治疗团队,为患者开展肿瘤康复药学服务。方法:临床药师通过建立肿瘤康复药学服务的整体策略和具体流程,规范药物治疗相关核心环节的服务方式,形成以肿瘤患者为中心的药学服务体系,包括肿瘤患者康复需求评估及计划制定、癌痛管理、营养支持、心理干预、药物不良反应监测、组织多学科会诊(MDT)等。结果:临床药师可通过在肿瘤康复过程中对肿瘤患者进行全方位的康复评估、用药管理和服务,为肿瘤患者制定规范化、个体化、连续性的综合康复治疗方案,促进肿瘤患者康复。结论:临床药师从肿瘤患者的康复需求出发提供药学服务,参与制定肿瘤患者个体化康复治疗方案,可切实提高肿瘤患者生命质量,实现药学服务的创新。Objective:To explore how to give full play to the professional advantages of clinical pharmacists to cooperate closely with the rehabilitation treatment team to carry out pharmaceutical care for tumor rehabilitation.Methods:By established the overall strategy and specific process of pharmaceutical care for tumor rehabilitation,clinical pharmacists standardized the service model of relevant core links of drug treatment and formed a pharmaceutical care system centered on tumor patients,including the assessment and planning of rehabilitation needs of tumor patients,cancer pain management,nutritional support,psychological intervention,adverse drug reaction monitoring,multi-disciplinary consultation(MDT)and so on.Results:Clinical pharmacists can make a comprehensive rehabilitation treatment plan for tumor patients,which is standardized,individualized and continuous,to promote the recovery of cancer patients.Conclusion:Clinical pharmacists should provide pharmaceutical care according to the rehabilitation needs of tumor patients and participate in the development of individualized rehabilitation treatment plan for tumor patients,which can effectively improve the quality of life of tumor patients and realize the innovation of pharmaceutical care.
