锌剂联合腹部按摩治疗婴幼儿腹泻临床研究

摘　　要：目的探讨锌剂联合腹部按摩治疗婴幼儿腹泻的临床疗效,以及对患儿胃肠道功能的调节作用。方法选取海南省海口市琼山区妇幼保健院2019年4月至2021年3月收治的婴幼儿腹泻患儿250例,随机分为观察组和对照组,各125例。两组患儿均予补液等常规治疗,对照组患儿加用蒙脱石散,观察组患儿加用锌剂、腹部按摩,均连续治疗7 d。结果观察组患儿总有效率为92.00%,显著高于对照组的78.40%(P<0.05)。治疗后,观察组患儿的胃排空率、胃窦收缩频率、胃体蠕动频率均显著低于对照组(P<0.05);观察组患儿的胃泌素、生长抑素、胃动素水平均显著低于对照组(P<0.05);观察组患儿的肿瘤坏死因子-α、D-乳酸、二胺氧化酶水平均显著低于对照组(P<0.05)。观察组和对照组治疗期间不良反应发生率相当(8.00%比12.80%,P>0.05)。结论锌剂联合腹部按摩治疗婴幼儿腹泻的临床疗效较好,能改善患儿的胃肠道功能,降低胃肠道激素和炎性因子水平,且治疗安全性良好。Objective To investigate the clinical efficacy of zinc preparations combined with abdominal massage in the treatment of infantile diarrhea and its regulation effects on gastrointestinal function.Methods A total of 250 infants with diarrhea admitted to the Maternal and Child Health Hospital of Qiongshan District from April 2019 to March 2021 were selected and randomly divided into the observation group and the control group,with 125 cases in each group.The infants in the two groups were treated with the routine treatment such as rehydration,on this basis,the infants in the control group were treated with Montmorillonite Powder,while the infants in the observation group were treated with zinc preparations and abdominal massage.Both groups were treated continuously for 7 d.Results The total effective rate in the observation group was 92.00%,which was significantly higher than 78.40%in the control group(P<0.05).After treatment,the gastric emptying rate(GER),antral contraction frequency(ACF)and gastric peristalsis frequency(GPF)in the observation group were significantly lower than those in the control group(P<0.05).The levels of gastrin(GAS),somatostatin(SS)and motilin(MOT)in the observation group were significantly lower than those in the control group(P<0.05).The levels of tumor necrosis factor-α(TNF-α),D-lactic acid(D-LA)and diamine oxidase(DAO)in the observation group were significantly lower than those in the control group(P<0.05).During the treatment,the incidence of adverse reactions in the observation group was similar to that in the control group(8.00%vs.12.80%,P>0.05).Conclusion Zinc preparations combined with abdominal massage is effective and safe in the treatment of infantile diarrhea,which can improve the gastrointestinal function,reduce the levels of gastrointestinal hormones and inflammatory factors.
