电针结合动态关节松动术治疗早中期股骨头坏死的临床观察

摘　　要：目的:观察电针结合动态关节松动术治疗早中期股骨头坏死的临床疗效。方法:将72例早中期股骨头坏死的患者随机分为治疗组(35例)和对照组(37例),治疗组患者予电针结合动态关节松动术治疗,对照组患者予电针结合功能锻炼治疗,观察并记录所有患者治疗前、治疗3个月后、6个月后的VAS评分及Harris髋关节功能评分,记录治疗过程中的不良反应,并随访观察复发情况。结果:两组患者治疗3个月后、6个月后VAS评分均低于治疗前(P<0.05),Harris评分均高于治疗前(P<0.05);治疗组患者治疗3个月后、治疗6个月后VAS评分均低于对照组(P<0.05),Harris评分均高于对照组(P<0.05);治疗组治疗3个月、6个月优良率均优于对照组(P<0.05)。随访6个月,治疗组复发病例2例,对照组复发病例5例。结论:电针结合动态关节松动术治疗早中期股骨头坏死,能有效缓解患者的疼痛,提高髋关节活动度,明显改善髋关节功能。Objective:To observe the clinical effect of electroacupuncture combined with mobilization with movement in the treatment of femoral head necrosis in the early and middle stages.Methods:A total of 72patients with femoral head necrosis in the early and middle stages were randomly divided into the treatment group(35 cases)and the control group(37 cases).The patients in the treatment group were treated with electroacupuncture combined with mobilization with movement,and the patients in the control group were treated with electroacupuncture combined with functional exercise.The VAS score and Harris hip function score of all patients before treatment,3 months after treatment,and 6 months after treatment were observed and recorded,adverse reactions during treatment were recorded,and recurrence was observed during follow-up.Results:The VAS scores of the two groups after 3 and 6 months of treatment were lower than those before treatment(P<0.05),and the Harris scores were higher than those before treatment(P<0.05).The VAS scores of the treatment group after3 months and 6 months of treatment were lower than those of the control group(P<0.05),and the Harris scores were higher than those of the control group(P<0.05).The excellent and good rates of the treatment group after3 and 6 months were better than those of the control group(P<0.05).Followed up for 6 months,there were 2cases of recurrence in the treatment group and 5 cases in the control group.Conclusion:Electroacupuncture combined with mobilization with movement in the treatment of femoral head necrosis in the early and middle stages can effectively relieve the pain of patients,improve the range of motion of the hip joint,and significantly improve the function of the hip joint.
