CT下“曲骨穴针刺至前列腺”治疗良性前列腺增生的临床研究

摘　　要：目的:评价CT下“曲骨穴针刺至前列腺”技术在临床治疗良性前列腺增生中的可行性。方法:选取良性前列腺增生患者20例,治疗前CT下测量曲骨穴针刺至前列腺的深度和角度;治疗时参考CT下测量的针刺深度和角度,针刺曲骨穴,使针尖沿耻骨后缘和膀胱间缝隙,触及前列腺包膜;治疗后CT下测量毫针实际针刺深度和角度,以及针刺后毫针偏离正中矢状面的距离和角度。并记录治疗前、治疗后CT下测量的针刺深度和角度,以及患者针刺安全性评价、针刺不适感视觉模拟评分、针刺承受性评分,评价临床治疗的安全可行性。结果:治疗前、治疗后CT下测量针刺深度比较,差异无统计学意义(P>0.05);治疗前、治疗后CT下测量针刺角度比较,差异有统计学意义(P<0.05);针刺后毫针偏离正中矢状面平均距离5.16 mm,偏离平均角度4.80°。结论:CT下“曲骨穴针刺至前列腺”技术治疗良性前列腺增生安全可行,患者易于接受。治疗前后针刺角度的差异,对治疗安全性无影响,原因在于针刺治疗时为了避免损伤膀胱,须结合解剖位置,不断调整针刺角度。Objective:To evaluate the feasibility of"acupuncture at Qugu point(CV2)to the prostate"under CT in the clinical treatment of benign prostatic hyperplasia.Methods:A total of 20 patients with benign prostatic hyperplasia were selected,and measuring the depth and angle of"acupuncture at Qugu point(CV2)to the prostate"with CT before treatment.During the treatment,referring to the acupuncture depth and angle measured under CT,acupuncture at Qugu point so that the needle tip touches the prostate capsule along the posterior border of the pubic bone and the gap between the bladder.The actual acupuncture depth and angle of the filiform needle,as well as the distance and angle of the filiform needle deviating from the midsagittal plane after acupuncture were measured under CT after treatment.The acupuncture depth and angle measured by CT before and after treatment were recorded,as well as the patient’s acupuncture safety evaluation,acupuncture discomfort visual analog score,and acupuncture tolerance score to evaluate the safety and feasibility of clinical treatment.Results:There was no significant difference in the acupuncture depth measured by CT before treatment and after treatment(P>0.05).Comparison of acupuncture angle measured by CT before treatment and after treatment,the difference was statistically significant(P<0.05).After acupuncture,the filiform needle deviates from the midsagittal plane by an average distance of 5.16 mm and an average angle of 4.80°.Conclusion:"Acupuncture at Qugu point(CV2)to the prostate"under CT is safe and feasible for benign prostatic hyperplasia,and the patients are easy to accept.The difference of acupuncture angle before and after treatment has no impact on the safety of treatment.The reason is that in order to avoid damaging the bladder,the acupuncture angle should be continuously adjusted in combination with the anatomical position.
