不同剂量泼尼松在肉芽肿性乳腺炎中的应用效果

摘　　要：目的探讨不同剂量泼尼松治疗肉芽肿性乳腺炎(GLM)的应用效果。方法选取2015年1月至2020年1月惠州市第二妇幼保健院乳腺科收治的100例GLM患者作为研究对象,采用随机数字表法将其分为试验组(n=50)和对照组(n=50)。试验组采用初始剂量0.4 mg/(kg·d)、随疗程递减(治疗5周后每周剂量降低2.5 mg/d)的泼尼松治疗方案,对照组采用初始剂量0.75 mg/(kg·d)、随疗程递减(治疗5周后每周剂量降低5 mg/d)的泼尼松治疗方案。两组患者均于治疗6~8周实施病灶清除术,比较两组患者的药物治疗效果、不良反应发生率、手术相关指标以及随访相关指标。结果两组的药物治疗有效率比较,差异无统计学意义(P>0.05);试验组的胃部不适、月经紊乱、血糖升高、失眠不安发生率低于对照组,差异有统计学意义(P<0.05)。两组的痤疮、体重增加发生率比较,差异无统计学意义(P>0.05);两组的术前服药时间、术中腺体切除量、治疗后痊愈时间比较,差异无统计学意义(P>0.05)。两组的术后并发症总发生率、随访1年复发率比较,差异无统计学意义(P>0.05)。结论不同剂量泼尼松治疗肉芽肿性乳腺炎患者在药物疗效方面并无差异,对患者后续手术治疗与复发率亦无影响。但小剂量泼尼松治疗肉芽肿性乳腺炎能有效减少患者的药物不良反应,值得深入研究。Objective To explore the application effect of different doses of Prednisone in the treatment of granulomatous mastitis(GLM).Methods From January 2015 to January 2020,100 patients with GLM admitted to the Department of Galactophore of Huizhou Second Maternity and Child Health Hospital were selected as the research objects,and they were divided into the experimental group(n=50)and the control group(n=50)by random number table method.The experimental group was treated with Prednisone with an initial dose of 0.4 mg/(kg·d)and decreasing with the course of treatment(the dose decreased by 2.5 mg/d per week after 5 weeks of treatment),and the control group was treated with Prednisone with an initial dose of 0.75 mg/(kg·d)and decreasing with the course of treatment(the dose decreased by 5 mg/d per week after 5 weeks of treatment).Both groups of patients were treated with debridement within 6 to 8 weeks of treatment.The effect of drug treatment and the incidence of adverse reactions,surgery-related indicators and follow-up related indicators were compared between the two groups.Results There was no significant difference in the effective rate of drug treatment between the two groups(P>0.05).The incidence rates of stomach discomfort,menstrual disorders,elevated blood sugar,and insomnia in the experimental group were lower than those in the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of acne and weight gain between the two groups(P>0.05).There was no significant difference in the preoperative medication time,intraoperative gland removal amount and recovery time after treatment between the two groups(P>0.05).There were no statistically significant differences in the total incidence of postoperative complications and the recurrence rate of 1 year follow-up between the two groups(P>0.05).Conclusion There is no difference in the drug efficacy of different doses of prednisone in the treatment of granulomatous mastitis,and it has no effect on the foll
