摘 要:目的分析妊娠期糖尿病(GDM)患者肠道菌群的变化及临床意义。方法选取2020年9月至2021年6月于深圳市宝安区石岩人民医院产检和分娩的100例GDM患者作为GDM组;另选取同期80例正常孕妇作为正常妊娠组,比较两组孕妇的肠道菌群及母婴不良妊娠结局,并根据GDM产妇的母婴妊娠结局分为正常分娩组与不良妊娠组,比较两组的肠道菌群检测结果。结果GDM组孕妇的肠杆菌、肠球菌菌落数大于正常妊娠组,双歧杆菌、乳杆菌菌落数小于正常妊娠组,差异有统计学意义(P<0.05)。GDM组孕妇的母婴不良妊娠发生率高于正常妊娠组,差异有统计学意义(P<0.05)。不良妊娠组孕妇的肠杆菌、肠球菌菌落数大于正常分娩组,双歧杆菌、乳杆菌菌落数小于正常分娩组,差异有统计学意义(P<0.05)。结论GDM患者存在肠道菌群紊乱,且母婴不良妊娠结局发生率较高;监测GDM患者肠道菌群变化,对临床防治GDM、改善母婴结局有一定的意义。Objective To analyze the changes and clinical significance of intestinal flora in patients with gestational diabetes mellitus(GDM).Methods A total of 100 GDM patients who underwent labor examination and delivery in Shiyan People′s Hospital of Bao′an District in Shenzhen City from September 2020 to June 2021 were selected as the GDM group.In addition,80 normal pregnant women in the same period were selected as the normal pregnancy group to compare the intestinal flora of the two groups of pregnant women and adverse pregnancy outcomes of their mothers and infants.According to the maternal and infant pregnancy outcomes of GDM women,they were divided into the normal delivery group and the adverse pregnancy group to compare the intestinal flora detection results of the two groups.Results The colonies of Enterobacter and Enterococcus in GDM group were higher than those in normal pregnancy group,while the colonies of Bifidobacteria and Lactobacillus were lower than those in normal pregnancy group,the differences were statistically significant(P<0.05).The incidence of adverse pregnancy in GDM group was higher than that in normal pregnancy group,and the difference was statistically significant(P<0.05).The colonies of Enterobacter and Enterococcus in the adverse pregnancy group were higher than those in the normal delivery group,while the colonies of Bifidobacteria and Lactobacillus were lower than those in the normal delivery group,the differences were statistically significant(P<0.05).Conclusion Patients with GDM have intestinal flora disorder,and the incidence of maternal and infant adverse pregnancy outcomes is higher.Monitoring the changes of intestinal flora in patients with GDM has certain significance for clinical prevention and treatment of GDM and improvement of maternal and infant outcomes.
