摘 要:糖尿病肾脏疾病(DKD)是慢性肾病的最常见病因之一,因此,为了延缓甚至逆转慢性肾病病情,需要更精确、更灵敏地早期诊断DKD。近年来,国内外学者通过酶联免疫吸附测定、蛋白质印迹等对DKD的生物标志物进行探究,在尿液、血液以及外泌体中寻求有利于DKD诊断、评估进展以及预后干预的新型分子标志物。DKD的发病机制较复杂,包括炎症反应、氧化应激以及纤维化等,发病机制尚未明确,而肾小管损伤在DKD的发生、发展过程中起着重要作用,甚至早于肾小球病变,发生于DKD的早期,因此对不同来源的肾小管相关分子指标的研究已成为DKD领域的关注热点。Diabetic kidney disease(DKD)is one of the most common causes of chronic kidney disease.Therefore,in order to delay or even reverse the course of chronic kidney disease,more accurate and sensitive early diagnosis of DKD is needed.In recent years,domestic and foreign scholars have been exploring the biomarkers of DKD by enzyme linked immunosorbent assay,Western blotting and other different methods,and seeking new molecular markers in urine,blood and exosomes which help to the diagnosis,evaluation progress and prognostic intervention of DKD.The pathogenesis of DKD is complex,including inflammation,oxidative stress,fibrosis,etc.,so the mechanism is not yet clear,.Renal tubular damage plays an important role in the in DKD occurrence and development,which occurs even before the glomerular lesions,in the early stage of DKD.Therefore the study of renal tubule related molecular indicators from different sources has become a focus in the field of DKD.
