大学新生2IU和5IU结核菌素纯蛋白衍生物皮肤试验结果分析

摘　　要：目的比较北京市大学新生使用2IU和5IU结核菌素纯蛋白衍生物(purified protein derivative of tuberculin,TB-PPD)进行皮肤试验的反应强度分布,为新生结核病筛查以及合理应用2IU、5IU TB-PPD提供参考。方法对北京市昌平区6所大学2017年入学新生采用整群抽样方法分为2组,其中3所学校新生使用2IU TB-PPD共5845人,3所学校新生使用5IU TB-PPD共5030人,比较2组基本特征和PPD皮肤试验反应结果,分析两种剂量下PPD皮肤试验的反应特点和局部不良反应情况。结果5IU组皮肤反应阳性率为33.0%,中度及以上阳性率为20.1%,强阳性率10.5%。2IU组皮肤硬结平均直径≥5 mm的阳性率为21.8%,≥10 mm的阳性率为12.3%,≥15 mm的阳性率为5.3%。2IU组皮肤硬结直径≥5 mm的阳性率与5IU组的中度及以上阳性率接近,2IU组皮肤硬结直径≥10 mm的阳性率与5IU组的强阳性率接近。5IU组出现局部不良反应比例高于2IU组。结论大学新生使用5IU TB-PPD进行皮肤试验的中度及以上阳性率、强阳性率分别为20.1%和10.5%;使用2IU TB-PPD进行皮肤试验的反应阳性率低于使用5IU TB-PPD;恰当使用结2IU TB-PPD和5IU TB-PPD可以达到相似的筛查效果。Objective Purpose To compare the reaction intensity distributions of freshmen in the university in Beijing using 2IU and 5IU purified protein derivative of tuberculin(TB-PPD)for skin tests,and provide a evidence for new tuberculosis screening and the rational application of 2IU and 5IU TB-PPD.Methods The 2017 freshmen of 6 universities in Changping district were divided into 2 groups using cluster sampling method.Among them,3 schools used 2IU TB-PPD for a total of 5845 freshmen,and 3 schools used 5IU TB-PPD for a total of 5030 freshmen.Compare the basic characteristics of the two groups and the PPD skin test.The reaction result and local adverse effects of PPD skin test under two doses were analyzed.Results In the 5IU group,the positive rate of skin reaction was 33.0%,the positive rate of moderate and above was 20.1%,and the positive rate of strong positive was 10.5%.In the 2IU group,the positive rate of skin induration≥5 mm was 21.8%,the positive rate of≥10 mm was 12.3%,and the positive rate of≥15 mm was 5.3%.The positive rate of skin induration≥5 mm in the 2IU group was close to the positive rate of moderate and above in the 5IU group,and the positive rate of skin induration≥10 mm in the 2IU group was close to the strong positive rate in the 5IU group.The proportion of local adverse reactions in the 5IU group was higher than that in the 2IU group.Conclusions The positive and strong positive rates of skin test for freshmen using 5IU TB-PPD were 20.1%and 10.5%,respectively;the positive rate of skin test using 2IU TB-PPD was lower than that of 5IU TB-PPD;using appropriate criterion of 2IU TB-PPD and 5IU TB-PPD can achieve similar screening result.
