摘 要:目的:探讨双平面Simpson法对冠心病(CAD)患者冠状动脉支架植入术(PCI)治疗前后左心室容积及收缩功能的评估价值。方法:选取284例行PCI术治疗的CAD患者为研究对象,分别于术前、术后1个月及术后3个月时行常规超声心动图检查,采用M型超声心动图Teichholz法(M型法)和双平面Simpson法分别测量并比较患者左心室容积和收缩功能。结果:术前,两种方法测量方法中左室收缩末容积(LVESV)、左室舒张末容积(LVEDV)和二尖瓣舒张早期最大峰值流速/二尖瓣舒张晚期最大峰值流速(E/A)值比较,差异有统计学意义(P<0.05);左心室射血分数(LVEF)值比较,差异无统计学意义(P>0.05)。术后1个月,两种测量方法中LVESV和LVEDV值比较,差异有统计学意义(P<0.05);LVEF和E/A值比较,差异无统计学意义(P>0.05)。术后3个月,两种测量方法中LVESV、LVEDV和E/A值比较,差异有统计学意义(P<0.05);LVEF值比较,差异无统计学意义(P>0.05)。Simpson法测量的心脏超声参数值与M型法测量值相关性均较高(P<0.05),其中LVEDV、LVEF及E/A值与M型法一致性较好(精确度均<30%),LVESV一致性欠佳(精确度>30%);Simpson法图像采集时间和图像分析时间长于M型法(P<0.05)。结论:应用双平面Simpson法能全面客观准确评价CAD患者PCI治疗前后的左心室容积及收缩功能,与M型法测值相关性高,可为临床诊断和预后评估提供更为精确的指导。Objective:To explore the evaluation value of left ventricular volume and systolic function by biplane Simpson method in patients with coronary heart disease(CAD) before and after percutaneous coronary intervention(PCI).Methods:284 patients with CAD who underwent PCI were selected as the study subjects.Conventional echocardiography was performed before surgery, 1 m after surgery and 3 m after surgery, respectively.M-mode echocardiography Teichholz method(M-mode method) and biplane Simpson method were used to measure and compare the left ventricular volume and systolic function.Results:Before operation, there were statistical differences in the left ventricular end systolic volume(LVESV),left ventricular end diastolic volume(LVEDV) and maximum peak velocity in early mitral valve diastole/maximum peak velocity in late mitral valve diastole(E/A) measured by Simpson method compared with those by M-mode method before surgery(P<0.05),but there was no statistical significance in LVEF value measured by the two methods(P>0.05).There were significant differences in the LVESV and LVEDV measured by Simpson method compared to M-mode method at 1 month after surgery(P<0.05),but there were no significant differences in LVEF and E/A measured by the two methods(P>0.05).The LVESV,LVEDV and E/A measured by Simpson method at 3 months after surgery were significantly different from those measured by M-mode method(P<0.05),but there was no statistical significance in the LVEF measured by the two methods(P>0.05).The cardiac ultrasound parameters values measured by Simpson method were highly correlated with measurement values by M-mode method(P<0.05),of which LVEDV,LVEF and E/A had good consistency with M-mode method(accuracy<30%) and LVESV had poor consistency(accuracy>30%).The image acquisition time and image analysis time of Simpson method were significantly longer than those of M-mode method(P<0.05).Conclusion:The application of biplane Simpson method can comprehensively, objectively and accurately evaluate the left ventricular volume
