头脊电针联合化痰通腑汤治疗中风后手指拘挛患者的临床效果

摘　　要：目的探讨头脊电针与化痰通腑汤联合治疗中风后手指拘挛患者的临床效果。方法选取2020年1月至2021年1月在哈尔滨市中医医院综合科诊治的180例中风后手指拘挛患者,按照随机数字表法将其分为三组,每组60例。电针组行头脊电针治疗,汤药组行化痰通腑汤治疗,联合组行头脊电针+化痰通腑汤治疗。观察患者治疗30 d后手部症状改善、运动功能障碍及临床效果。结果治疗后,三组手指痉挛评分和手神经功能缺损评分均低于治疗前,且联合组低于电针组和汤药组,差异有统计学意义(P<0.05);治疗后,三组手功能评分均高于治疗前,且联合组高于电针组和汤药组,差异有统计学意义(P<0.05)。治疗后,三组运动协调能力评分、感觉功能评分及总评分均高于治疗前,且联合组高于电针组和汤药组,差异有统计学意义(P<0.05)。治疗后联合组临床效果优于电针组和汤药组,差异有统计学意义(P<0.05)。结论中风后手指拘挛临床治疗中同时行头脊电针治疗和口服化痰通腑汤治疗,可有效提高临床治疗总效果。Objective To investigate the clinical effect of head-spine electroacupuncture combined with Huatan Tongfu Decoction on finger spasm after stroke.Methods A total of 180 patients with finger spasm after stroke treated in Department of General,Harbin Traditional Chinese Medicine Hospital from January 2020 to January 2021 were selected and they were divided into three groups according to random number table method,with 60 patients in each group.The electroacupuncture group was treated with head-spine electroacupuncture,the decoction group was treated with Huatan Tongfu Decoction,and the combined group was treated with head-spine electroacupuncture combined with Huatan Tongfu Decoction.The improvement of hand symptoms,dyskinesia and clinical effect were observed after 30 days of treatment.Results After treatment,the scores of finger spasm and hand nerve function defect in three groups were lower than before,and those in the combined group were lower than those in electroacupuncture group and decoction group,the differences were statistically significant(P<0.05);after treatment,the score of hand function in the three groups was all higher than before,and that in the combined group was higher than in the electroacupuncture group and decoction group,the differences were statistically significant(P<0.05).After treatment,the motor coordination ability score,sensory function score and total score in the three groups were higher than those before treatment,and those in the combined group were higher than those in electroacupuncture group and decoction group,the differences were statistically significant(P<0.05).After treatment,the clinical effect of combined group was better than electroacupuncture group and decoction group,and the difference was statistically significant(P<0.05).Conclusion In the clinical treatment of finger spasm after stroke,simultaneous treatment with head-spine electroacupuncture combined with Huatan Tongfu Decoction can effectively improve the total clinical effect.
