注射用盐酸万古霉素致不良反应的临床分析

摘　　要：目的减少盐酸万古霉素不良反应的发生,为临床合理用药提供建议。方法通过查阅药品说明书和参考文献,对2例注射用盐酸万古霉素致不良反应案例进行了关联性评价与分析。结果2例不良反应的发生与使用盐酸万古霉素的关联性评价均为“很可能”,不良反应的发生与用药情况及患者自身体质等因素有关。结论使用注射用盐酸万古霉素,要严格控制溶媒量和药物浓度及滴注速度;联合用药可能会增加盐酸万古霉素不良反应发生的风险;在治疗过程中应监测血药浓度,尽量保证特殊群体的个体化给药;注意加强特殊体质者药物使用监测。Objective To reduce the occurrence of adverse drug reactions of vancomycin hydrochloride for injection and provide suggestions for rational drug use in clinic.Methods 2 cases of adverse drug reactions induced by vancomycin hydrochloride for injection were evaluated and analyzed by consulting the drug directions and references.Results The correlation between the occurrence of 2 ADR cases and the use of Vancomycin hydrochloride for injection was“Very likely”.The occurrence of ADR was related to the use of drugs and the patients’physical constitution.Conclusion The dosage and concentration of vancomycin hydrochloride for injection should be strictly controlled,and the rate of infusion should be controlled.The risk of adverse drug reactions about vancomycin hydrochloride for injection would be increased by the combination of other drugs,and the blood concentration of vancomycin hydrochloride should be monitored during treatment to ensure the individualized administration of drugs to special groups as far as possible,and pay attention to strengthen the monitoring of the use of special constitution.
