贝那普利联合PDE抑制剂治疗顽固性心力衰竭的效果及对心功能和血清炎症因子表达的影响

摘　　要：目的探究贝那普利联合磷酸二酯酶(PDE)抑制剂治疗顽固性心力衰竭的效果及对心功能和血清炎症因子表达的影响。方法选取2019年1月~2020年12月的200例顽固性心力衰竭的患者,采用随机分组法分为观察组与对照组,各100例患者。对照组使用贝那普利片治疗顽固性心力衰竭,观察组则在对照组的基础上联合PDE抑制剂米力农治疗顽固性心力衰竭。对比两组患者治疗前后的心功能和血清炎症因子的表达水平。结果两组患者治疗前心功能指标差异无统计学意义(P>0.05)。在两组患者治疗5 d后,两组患者的心功能指标LVESd、LVEDd均较同组治疗前显著下降(P<0.05),LVEF上升(P<0.05),且观察组治疗后LVESd、LVEF、LVEDd水平显著优于对照组(P<0.05)。两组患者治疗前血清中的AngⅡ、NT-proBNP、sST2水平无明显差异,差异无统计学意义(P>0.05)。在两组患者治疗5 d后血清中的AngⅡ、NT-proBNP、sST2水平较治疗前有显著的降低,观察组的AngⅡ、NT-proBNP、sST2水平显著低于对照组(P<0.05)。两组患者在接受治疗前,血清中IL-33、TNF-α、ICAM-1的指标无明显差异(P>0.05)。在两组患者接受治疗5 d后,血清中IL-33、TNF-α、ICAM-1的指标较治疗前有显著的降低(P<0.05)。治疗后的观察组与对照组相比较IL-33、TNF-α、ICAM-1的水平有显著降低(P<0.05)。两组患者治疗前IVS、LVPW、LVMI心室重构比较无明显差异,治疗后,两组患者的IVS、LVPW、LVMI都发生了显著减小(P<0.05),且观察组IVS、LVPW、LVMI与对照组相比较显著减小(P<0.05)。观察组的总有效率显著高于对照组(P<0.05)。结论贝那普利联合PDE抑制剂治疗顽固性心力衰竭有明显的效果,对心功能的恢复和血清炎症因子的降低有显著的改善。Objective To investigate the effect of benazepril combined with phosphodiesterase(PDE)inhibitor on cardiac function and serum inflammatory factors in the treatment of refractory heart failure.Methods A total of 200 patients with refractory heart failure in our hospital from January 2019 to December 2020 were enrolled,and randomly divided into two groups,each with 100 cases.The control group received benazepril,while the observation group received benazepril combined with PDE inhibitor.Then the cardiac function related indicators and expressions of serum inflammatory factors were compared between the two groups.Results The Values of left ventricular end-systolic diameter(LVESd),left ventricular ejection fraction(LVEF)and left ventricular end-diastolic diameter(LVEDd)showed no significant difference between the two groups at baseline(P>0.05),and those values of LVESd and LVEDd were significantly decreased in both groups after treatment(P<0.05),and were significantly lower in observation group than in control group(P<0.05).And the LVEF of observation group was better than that of the control group(P<0.05).Serum levels of angiotensin Ⅱ(AngⅡ),soluble growth stimulation-expressed gene 2(ST2)and N-terminal pro-brain natriuretic peptide(NT-proBNP)showed no significant difference between the two groups at the baseline(P>0.05),which were significantly decreased in both groups after treatmen(t P<0.05),and the decrease was more obvious in observation group than in control group(P<0.05).Levels of interleukin-33(IL-33),tumor necrosis factor-α(TNF-α)and intercellular adhesion molecule-1(ICAM-1)showed no significant difference between the two groups at the baseline(P>0.05),which were significantly decreased in both groups after treatment(P<0.05),and the decrease was more significant in observation group than in control group(P<0.05).The interventricular septal thickness(IVS),left ventricular posterior wall thickness(LVPW)and left ventricular mass index(LVMI)showed no significant difference between the two groups at the bas
