自制水囊在剖宫产宫缩乏力出血中的临床应用

摘　　要：目的:探讨自制水囊在剖宫产宫缩乏力出血的应用效果。方法:选择2010年8月-2021年10月昌平区中西医结合医院收治的97例剖宫产术中宫缩乏力性出血患者,随机分成两组,试验组49例给予宫腔压迫水囊,对照组48例给予宫腔纱条填塞,比较两组患者的术后1 h、2 h、4 h、24 h产后出血量,临床效果、取出水囊、纱条情况及用药不良反应发生率。结果:产后出血量试验组明显少于对照组,而止血总有效率明显高于对照组,差异均有统计学意义(P<0.05);试验组疼痛、取出困难、取出时出血量>10 mL的例数少于对照组,差异均有统计学意义(P<0.05),试验组不良反应发生率与对照组相比,差异无统计学意义(P>0.05)。结论:自制水囊压迫止血治疗剖宫产术中宫缩乏力性出血具有止血效果好、疼痛少、取出容易的优点,疗效显著,值得推广。Objective:To explore the application effect of self-made water sac in uterine inertia and bleeding during cesarean section.Methods:A total of 97 patients with uterine inertia and bleeding during cesarean section treated in Changping District Hospital of Integrated Chinese and Western Medicine from August 2010 to October 2021 were selected randomly divided into two groups.The 49 cases in the experimental group were given intrauterine compression water sac and 48 cases in the control group were given intrauterine gauze packing.The postpartum hemorrhage volume at 1 h,2 h,4 h,and 24 h after operation,clinical effect,removal of water sac and gauze,and incidence of adverse drug reactions were compared between the two groups.Results:The amount of postpartum hemorrhage in the experimental group was significantly less than that in the control group,and the total effective rate of hemostasis was significantly higher than that in the control group.The differences were statistically significant(P<0.05).The number of cases with pain,removal difficulty and bleeding volume>10 ml upon removal in the experimental group were lower than those in the control group,and the differences were statistically significant(P<0.05).The incidence of adverse reaction showed no significant difference between two groups(P>0.05).Conclusion:Self-made water sac compression hemostasis in the treatment of uterine inertia and bleeding during cesarean section has the advantages of good hemostatic effect,less pain and easy removal.It has significant curative effect and is worthy of popularization.
