摘 要:目的:本实验为探讨抗C1q抗体及抗dsDNA抗体联合检测系统性红斑狼疮疾病进展程度(SLEDAI)的相关性及预测价值。方法:选取2018年1月至2019年9月就诊于中国医科大学附属盛京医院风湿免疫科收治的住院患者及同一时间段本院体检中心健康体检人群共计298例,采取连续入组的方法。其中系统性红斑狼疮(SLE)患者100例,其他自身免疫病患者98例,体检患者100例。测定抗C1q抗体及抗dsDNA水平,分析抗C1q抗体和SLEDAI及实验室指标的关系。结果:低活动组抗C1q抗体(P=0.01)、抗dsDNA抗体(P<0.0001)水平低于中高活动组,补体C3(P<0.0001)、C4(P=0.03)、C-反应蛋白高于中高活动组;SLEDAI与病程、补体C3、C4呈负相关,与白细胞、年龄、抗C1q抗体(P=0.008)、抗dsDNA抗体呈正相关(P<0.05);抗C1q抗体评估SLE疾病活动度的敏感性和特异性为75.90%和71.40%;抗dsDNA抗体评估SLE疾病活动度的敏感性和特异性为100.00%和42.90%。结论:抗C1q抗体联合抗dsDNA抗体检测对SLE患者疾病预测及狼疮性肾炎患者病理类型预测和治疗效果评估均有重要临床意义。Objective:This study was to investigate the correlation and predictive value of anti-C1q anti-body and anti-dsDNA antibody combined detection of systemic lupus erythematosus disease progression(SLEDAI).Methods:A total of 298 inpatients admitted to the Department of Rheumatology and Immunology of Shengjing Hospital of China Medical University from January 2018 to September 2019 and healthy people in the physical examination center of our hospital during the same period were selected,and the method of continuous enroll-ment was adopted.There were 100 patients with SLE,98 patients with other autoimmune diseases and 100 healthy subjects.The levels of anti-C1q antibody and anti-dsDNA were measured,and the relationship between anti-C1q antibody and SLEDAI and laboratory indicators was analyzed.Results:The levels of anti-C1q antibody(P=0.01)and anti-DSDNA antibody(P<0.0001)in the low-activity group were lower than those in the medium-high activity group,and the levels of C3(P<0.0001),C4(P=0.03)and CRP were higher than those in the medium-high activi-ty group.SLEDAI was negatively correlated with the course of disease,C3 and C4,but positively correlated with leukocyte,age,anti-C1q antibody(P=0.008)and anti-dsDNA antibody(P<0.05).The sensitivity and specificity of anti-C1q antibody in evaluating SLE activity were 75.9%and 71.4%,respectively.The sensitivity and specificity of anti-dsDNA antibody in evaluating SLE disease activity were 100.0%and 42.9%respectively.Conclusion:An-ti-C1q antibody combined with anti-dsDNA antibody detection has important clinical significance for disease pre-diction of SLE patients and pathological type prediction and therapeutic effect evaluation of lupus nephritis patients.
