基于结构-过程-结果三维质量评价模式的延伸护理在尿毒症血液透析患者中的应用效果

摘　　要：目的:观察基于结构-过程-结果三维质量评价模式的延伸护理在尿毒症血液透析患者中的应用效果。方法:回顾性分析2018年1月至2021年1月该院收治的87例尿毒症血液透析患者的临床资料,依据护理方案不同分为对照组(43例)和观察组(44例)。对照组采用常规延伸护理,观察组采用基于结构-过程-结果三维质量评价模式的延伸护理。比较两组护理前后成人健康自我管理能力测评表(AHSMSRS)评分、应对方式评分,以及并发症发生率。结果:护理后,观察组AHSMSRS评分、积极应对评分高于对照组,消极应对评分低于对照组,差异均有统计学意义(P<0.05);观察组并发症发生率为4.55%(2/44),低于对照组的20.93%(9/43),差异有统计学意义(P<0.05)。结论:基于结构-过程-结果三维质量评价模式的延伸护理可提升尿毒症血液透析患者的健康自我管理能力,降低并发症发生率,促使其由消极应对态度转变为积极应对态度,效果优于常规延伸护理。Objective:To observe application effects of extended nursing based on structure-process-result three-dimensional quality evaluation model in uremic hemodialysis patients.Methods:The clinical data of 87 uremic hemodialysis patients admitted to our hospital from January 2018 to January 2021 were retrospectively analyzed.According to different nursing programs,they were divided into control group(43 cases)and observation group(44 cases).The control group received conventional extended nursing,while the observation group was give the extended nursing based on the structure-process-result three-dimensional quality evaluation model.The scores of health self-management ability[adult health self-management ability self-rating scale(AHSMSRS)],the coping style scores and the incidence of complications were compared between the two groups before and after the nursing.Results:After the nursing,the AHSMSRS score of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).After the nursing,the positive coping score of the observation group was higher than that of the control group;the negative coping score was lower than that of the control group;the differences were statistically significant(P<0.05).Further,the incidence of complications in the observation group was 4.55%(2/44),which was lower than 20.93%(9/43)in the control group,and the difference was statistically significant(P<0.05).Conclusions:The extended nursing based on the structure-process-result three-dimensional quality evaluation model can improve the health self-management ability of the uremic hemodialysis patients,reduce the incidence of complications,and promote the change from a negative coping attitude to a positive coping attitude.Moreover,it is superior to the conventional extended nursing.
